- Price Schedule Scope of Work for Cylinders. Scope of Work for Bulk Oxygen. This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Navajo Area Indian Health Services, Chinle Comprehensive Health Care Facility (CCHCF) is requesting for submittal of offers for a contractor to deliver and refill bulk and cylinder medical gas on an as needed basis to the Navajo Area Indian Health Service, Chinle Comprehensive Healthcare Facility (CCHCF), Off Highway 191 & Hospital Drive, Chinle, AZ 86503. Services shall be performed in accordance with the attached Scope of Work (SOW). Period of Performance: Date of Award through 09/30/2018. The Government will consider competitive rates upon negotiation. The Chinle Comprehensive Health Care Facility (CCHCF) is based in Chinle, Arizona (Northeast Arizona near Canyon De Chelly National Monument). The CCHCF is a 60 bed hospital which serves as the health care hub for the region. The medical staff includes Family Physicians, Internists, Pediatricians, General Surgeons, OB/GYN's, Anesthesiologists, and a Psychiatrist. In addition to routine outpatient and inpatient primary care, services available to our patients include: Adult Intensive Care, General Surgery (including laparoscopic surgery), routine and operative Obstetrics, and 24-Hour Emergency Room Services. Health care services are provided to approximately 37,000 active users. SUBMITTAL PROCESS: To be considered for award, your offer shall include the following documents: 1. Pricing Schedule for the following: a. Price for various size cylinders for in-house. b. Price for Medical Liquid Bulk Oxygen. 2.
