Description
W81K02-18-T-0030 formerly W81K02-17-T-0263 Notice of Intent for a Sole Source for Lab Specimen Testing Services MEDCOM, Joint Base Lewis McChord Health Contracting Office (JBLM-HCC), WA intends to award a firm fixed priced order under FAR Part 6.302-1 for Lab Specimen Testing Services to Foundation Health, LLC (doing business as Fairbanks Memorial Hospital (FMH)) in the amount of Not To Exceed $150,000.00 for a base year with three option year periods. These testing services will include but not be limited to: blood, serum, urine, tissue, feces, mucosal, body fluids and spinal fluids prescribed by the patients' physician/surgeon and determined by the physicians to best meet the medical, health and life-style needs of specific patients for whom they are required. Bassett Army Community Hospital (BACH) Pathology Department is located in the interior of Alaska and FMH is located ten miles from BACH, while the next closest hospital is located 358 miles away from BACH. Due to the turnaround time required (within 2 hours) to obtain test results by the physicians for patient care and patient safety, this requirement is critical to the hospital. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under authority of FAR 6.302-1. Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice is not a request for proposal and a solicitation will not be issued in conjunction with the notice of intent. Any vendor that believes it can provide the stated supplies/equipment can provide supporting documents demonstrating their products capability and specifications. Responses received will be used for future acquisitions. Information can be sent to the Contract Specialist; Ms. Jasmine DeVinney at jasmine.l.devinney.civ@mail.mil. No telephone or facsimile requests will be accepted.
