Please fill out page 1 and attach the document to your quote List of Medical Supplies Provisions for the Solicitations Synopsis: This is a combined synopsis/soloicitation for various Medical Supplies conducted under Simplified Acquisition Procedures (SAP) pursuant to the authority of FAR Subpart 13. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The current solicitation number is: 18-239-SOL-00007 and is issued as a Request for Quotation (RFQ). The clauses and provisions referenced in this solicitation may be obtained in full text form at http:/www.arnet.gov/far The Department of Health and Human Services/Indian Health Service has a requirement for various medical supplies. Information is on attached list of requested office supplies for the Bemidji Area Indian Health Service. Delivery/Ship to location: The Red Lake Indian Hospital, located in Red Lake, Minnesota. POC for this buy is Christopher.Millard@IHS.gov Set-Aside This is set-aside 100% for Indian Economic Enterprise small business vendors only pursuant to 25 USC 47, the Buy Indian Act. Basis for award: The Government intends to award a single award resulting from the solicitation to the responsible offerors whose proposal represents lowest acceptable cost. Proposals shall contain the offerors best pricing. The Government intends to evaluate and award with no discussion. The Government reserves the right to make no awards as a result of this solicitation The associated North American Industrial Classification System (NAICS) code for this procurement is 339113 with the size standard of 750. **All proposals are to be submitted electronically to Christopher.Millard@IHS.gov.
