- This amendment is to reopen this solicitation for offers. The period of performance has been changed from base plus two option years to base plus one option year. Please see attached documents for amended periods of performance. The new deadline for questions concerning this solicitation is 11:00am Mountain Standard time on 1 December 2017. The new deadline to submit offers is 11:00am Mountain Standard Time on 5 December 2017. Combined Synopsis/Solicitation W911RZ-18-Q-0004 Hospital Catholic Priest Service This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Quotes shall reference solicitation number is W911RZ-18-Q-0004. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95, dated 13 January 2017. The North American Industrial Classification System (NAICS) code for this procurement is 813110, standard size $7.5 Million. This requirement is "unrestricted". Fort Carson intends to award a firm-fixed-price purchase order for Catholic Priest Services. The services shall be performed in accordance with the attached Performance Work Statement. A priest wishing to serve as a contract chaplain with the Archdiocese Military Service (AMS), must apply for and receive from the Military Ordinary the appropriate endorsement before AMS faculties may be granted and provide written proof to the Contracting Officer of permission from his diocesan bishop or religious superior to enter into these services as well as provide current Ecclesiastical Endorsement from the Archdiocese for Military Services (AMS), USA http://www.milarch.org prior to award. The contract will be written for a base year plus a one year option period. The Base Year period of performance (POP) is 11 December 2017 through 10 December 2018, and Option Year OY 1 from 11 December 2018 through 10 December 2019.
