- Attachment 1 Attachment 2 RFQ 3 (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a separate written request for quotation will not be issued. This combined synopsis/solicitation will result in firm fixed-price line items in a requirements-type contract with a base and _4_ option years, unless extended or terminated. (ii) The reference number is _12639518R0006___ and the solicitation is issued as a request for quote (RFQ). The SF-1449 Form and the detailed terms and conditions are presented in Attachment to this combined synopsis/solicitation. (iii) The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulation (FAR) and Agriculture Federal Acquisition Regulations supplement (AGARs) can be accessed on the Internet at http://www.acquisition.gov/far and http://www.usda.gov/procurement/policy/agar.html. (iv) It is anticipated that either a single award will be awarded. This requirement is set aside strictly for small business. The associated NAICS code for this effort is __541380__ and small business size standard is _$15.0 million_. (v) This requirement consists of ground water and waste water sampling and analysis per the specifications in the SOW. The vendor shall return a Price Quotation with offered prices along with other requested information to the contracting specialist.. The government anticipates pricing for the 5-year requirement to be somewhere between $30,000 to $50,000. (vi) Delivery and acceptance of deliverables will be FOB destination. (vii) The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this solicitation. (viii) The provision at 52.212-2, Evaluation - Commercial Items, applies to this solicitation. The following text is added to Paragraph (a) of FAR 52.212-2: (a) The Government will award a contract resulting from this solicitation to the responsible firm(s) with the lowest priced technically acceptable (LPTA) quote based on the lowest evaluated price of proposals meeting or exceeding the acceptability standards: technical capability based on the SOW, and past performance. Additional guidance on price and past performance is provided under provision 52.212-1. Offerors with no past performance will be rated neutral. (ix) The provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, applies to this solicitation.. A copy of the provision may be attained from http://www.acquisition.gov/far/ (x) The clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. FAR clauses and AGAR clauses identified in Attachment are considered applicable to this acquisition. (xi) The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The checked FAR clauses are identified in the Attachment. (xii) To be awarded this contract, the offeror must be registered with current information in the SAM database. SAM information may be found at http://www.sam.gov. (xiii) The Defense Priorities and Allocations System (DPAS) is not applicable to this acquisition. (xiv) No applicable Numbered Notes.
