- FAR 52.209-2, FAR 52.225-25, and FAR 52.222-22 Draft BPA Combined Synopsis Solicitation Performance Work Statement The Government, specifically the Medical Treatment Facility (88 MDSS/SGSM) located at Wright-Patterson AFB, OH has the requirement Hemoglobin/Lead Blood Testing. The following is the list of requirements: •1. The contractor will be responsible for providing lead and hemoglobin filter paper kits; any such items, devices or supplies that are used solely to collect, transport, process or store specimens to be submitted for testing and report test results to laboratory facility within 24 hours once received in the testing facility via fax or online service available for laboratory personnel to log onto to retrieve patient results. •2. The lead and hemoglobin (both tests must be available) filter paper kit requires collection to be completed by capillary stick only. Averaging around 40 kits per month, but not limited to or guaranteed to be 40 kits per month. •3. The contractor shall protect the privacy, integrity, security and confidentiality of all protected health information (PHI) disclosed under HIPAA privacy regulation laws. •4. Contractor shall have licensure/certification with the College of American Pathologists, and Health Care financing agencies such as Medicaid and the Clinical Laboratory Improvement Amendments (CLIA) Program, and State licensure where required. Any secondary laboratory used by the contractor to provide services to WPAFB shall also have appropriate licensure/certifications. •5. The contractor shall be responsible for creating, maintaining, and disposing of only those government required records that are specifically cited in this PWS. If requested by the Government, the contractor shall provide the original record or a reproducible copy of any such record within five working days of receipt of the request. The Government may award one or more BPAs resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Technical Capability and Price will be used to evaluate all offers.
