Description
The W. G. Hefner Veterans Medical Center Salisbury, North Carolina and its associated health care centers have a need to have the annual calibration, and performance and quality control testing of all diagnostic x-ray equipment. The Medical Centers also have a need to have their quarterly Nuclear Medicine inspections. PLACE OF PERFORMANCE: W. G. Hefner Medical Center, 1601 Brenner Ave. Salisbury, NC 28144 South Charlotte Veterans Health Care Center, 3506 W Tyvola Rd, Charlotte, NC 28208 VA Outpatient Clinic, 8601 University E Drive, Charlotte, NC 28213 Kernersville VA Health Care Center, 1695 Kernersville Medical Pkwy, Kernersville, NC 27284 Annual calibration, performance and quality control testing on all diagnostic x-ray equipment to include radiation safety testing and regulatory compliance testing and imaging performance of the equipment. Annual inspection will be for the imaging devices listed below in accordance with American College of Radiology (ACR) & Joint Commission requirements. Physicists must be current on all American College of Radiology (ACR) & Joint Commission certification requirements. Nuclear Medicine Quarterly license support/inspections will be completed once per quarter at Salisbury VAMC, Charlotte Health Care Clinic, and the Kernersville Health Care Clinic. Once inspections have been completed contractor must provide biomedical personnel with a hard and electronic copy of the inspection report to include all measurements and results. Contractor must check in with biomedical personnel at the beginning and end of each day and provide a daily schedule in advance to ensure there will be no patient flow disruption during the inspection. The vendor will work with the Individual site leads to determine when inspections are to be completed. For additional new equipment and re-certification s, the vendor is to be able to provide support within 5 business days after VA has made contract with Vendor. Items to be included for Services Quarterly Nuclear Medicine Hot lab quality control inspections Annual Nuclear Medicine program review Annual Inspection on lead aprons, lead gloves and thyroid collars. (365 total items) 2 Additional new equipment certifications (value at highest cost modality, Magnetic Resonance Imaging (MRI) 1 Recertification for post equipment repair: Mammography 2 Recertification for post equipment repair: X-Ray 1 Recertification for post equipment repair: Computerized Tomography (CT) All Annual ACR/Joint Commission Inspections of the equipment below The expected period of performance is from award date to 30 September 2018 for the Base Year of the contract follow by four (4) one (1) Option Year plus one (1) six (6) months extension periods to be exercised at the Government discretion. It is the VA intention to negotiate and award on a competitive basis. The NAICS code for this requirement is 541380. Size standard is $15M. This is a source sought notice only. No formal solicitation document exists at this time. Responses to this sources sought notice should include the following: Name and address of company; business size (large or small); identify if firm falls into any of the socioeconomic categories and any pertinent information which demonstrates the firm s ability to meet the above requirement.
