- Attachment A Price list W81K02-18-T-0041Att1 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Regional Health Contracting Office - Pacific, Bldg 339 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, and award a Blanket Purchase Agreement for immunohematology blood bank reagents, brand name or equal for the Brian Allgood Army Community Hospital in Yongsan, Korea. This is a request for quotation (RFQ). This solicitation number is W81K02-18-T-0041. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-96. The Standard Industrial Classification Code is 6550, size standard is 1,250 employees and the NAICS code of 325413. This procurement is unrestricted. Provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition and a statement regarding any addenda to the provision. See W81K02-18-T-0041Att1. Provision at 52.212-2, Evaluation -- Commercial Items, and its addendum applies to this acquisition. W81K02-18-T-0041Att1. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its addendum applies to this acquisition. W81K02-18-T-0041Att1. The deviation to clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition W81K02-18-T-0041Att1 for the FAR clauses that are applicable to this acquisition. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award a Blanket Purchase Agreement for up to five years. Contract Requirements - The successful offeror(s) shall be required to provide the immunohematology blood bank reagents with the minimum requirements given in the attachment; W81K02-18-T-0041Att1. Contractors shall quote their unit pricing for the items listed in the Description of Requirement or a price list that meets the salient characteristics of each item listed in the Description of Requirement.
