Description

The Department of Veterans Affairs, Network Contracting Office (NCO) 16, intends to award a Sole Source, Blanket Purchase Agreement to Quest Medical/ Arthrex to ensure that the Southeast Louisiana Veterans Health Care System (SLVHCS) is covered for general surgery, vascular and orthopedics for the period of performance. The proposed service is to provide the equipment, labor, materials, warranty, installation, user training, accessories, scopes (rigid and flexible), and supplies necessary to provide a Complete Laparoscopic Operating System located at SLVHCS, 2400 Canal Street, New Orleans, LA 70119. The period of performance is (6) six months from January 1, 2017 through June 30, 2018 during scheduled intervals on a weekly basis (24 hours/week) consistent with the operations of the Medical Center, manufacturer s recommended specifications, and the equipment s seasonal service operational requirements. SLVHCS hours of operation are 7:30am 7:00pm Monday-Friday excluding federal holidays. All materials and services provided shall be in accordance with the OEM specifications and guidelines. During schedule service intervals, all parts including batteries and accessories shall be replaced at no additional cost to the Government. Unscheduled and scheduled travel are included as part of the service contract. Award will be made on or about December 18, 2017. The NAICS Code is 339112 surgical and medical instrument manufacturing. This notice is NOT a request for quote, a solicitation of offers, or an invitation for bid. The Government does not anticipate receiving responses to this notice; however, all responsible sources may submit a capability statement, proposal, or quote to aaron.angstadt@va.gov, which may be considered by the agency.
