Sources Sought: This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 621112, Offices of Mental Health Physician, Size Standard $11.0 Million. The Network Contracting Organization at Canandaigua (VISN 2) has a requirement for a contractor to provide on-site psychological and medical compensation and pension exams for veterans at the Rochester Outpatient Clinic, Rochester, New York, 14624. Responses to this sources sought must include the following information. 1. General information of your company name, address, contact person name, phone number, fax number, email address(s) and DUNS number and company website if available. 2. Type of business such as 8(a), small business, Hub Zone small business, Small Disadvantaged Business, Service Disabled Veteran Owned small business or Veteran owned small business. 3. Company experience with a Department of Veterans Affairs compensation and pension exam program. Responses to this market survey should be in writing and shall be e-mailed to the attention of Jeanette Crooks, Contract Specialist at Jeanette.Crooks@va.gov. No telephone responses will be accepted. Responses must be received no later than 4:30 pm December 22, 2017. No solicitation document is available at this time; this notice is to acquire information only.
