- This is a sources sought notice only and it is not a solicitation announcement. The Lincoln, NE Veterans Administration anticipates entering into a base year and four option year firm fixed price service contract with a contractor to provide cable tv services as specified below. The purpose of this sources sought notice is to gain knowledge of qualified potential contractors and their socio-economic size classification in accordance with NAICS code 515210. Responses will be used by the Government to make appropriate acquisition decisions. After a review of the responses to this sources sought notice, a solicitation announcement may be posted to Federal Business Opportunity website (www.FBO.gov). A response to this sources sought notice is not an adequate reply to the forthcoming solicitation. STATEMENT OF WORK Contractor shall provide Cable Television services for VA Nebraska Western Iowa Health Care System, (VA NWIHCS) Lincoln Division, 600 S. 70th Street, Lincoln, Nebraska. The television service at the VA Medical Center in Lincoln, NE will include a minimum of 60 channels, and channel lineup must include the following channels. Within three months of contract award, the contractor shall activate cable services, such that all 50 TVs are live with video from the contractor s designated cable provider. REQUIREMENTS - GENERAL: Contractor will provide the following services based on the following requirements: Contractor will provide all tools, transportation, materials and labor to connect their plant to existing RG-6 cable drops going to various patient and waiting rooms. Installation must meet the general requirements of Commercial Building Telecommunications Standards set forth by OSHA, TIA/EIA and all NFPA codes. All fire wall penetrations must be secured properly with Specseal intumescent firestop. Any necessary cosmetic work to facility or other damage is the responsibility of the contractor. All installation work shall be performed during normal business hours (7am-4pm CST) unless the contractor submits a request to the COR prior to the proposed start of the work. Contractor must limit the spread of dust, dirt, and germs during cable placement and construction. Contractor is required to provide and use a HEPA filtered vacuum to frequently clean the worksite. Proper disposal of trash is also required. Contractor will coordinate work with Contracting Officers Technical Representative as identified by attachments provided at the time of award of the contract. SERVICES: a. The services specified in the Sections entitled Schedule of Supplies/Services, Statement of Work and Special Contract Requirements may be changed by written modification to this contract. The modification will be prepared by the VA Contracting Officer and, prior to becoming effective, shall be approved by the contractor as part of the bi-lateral modification process. c. The services to be performed by the contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by laws of the VA facility. d. Under no circumstances will Contractor s employees be considered Lincoln VAMC employees. e. Contractor will hold certificate of insurance for general liability, workmen s compensation and vehicle insurance. Prior to award of the contract the Contractor shall furnish to the VA a certificate of insurance evidencing that all required coverage has been obtained. The Contractor shall be responsible for maintaining this certificate/coverage for the duration of the contract. QUALIFICATIONS: Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All certifications/licenses held by the personnel working on this contract shall be full and unrestricted. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. Each person assigned to work under this contract shall be certified and trained in cable television installation and repair services and operation of any equipment used. APPROVAL OF CONTRACTOR PERSONNEL: The Department of Veterans affairs reserves the right to refuse employment under this contract or require dismissal from contract work of any contractor employee or subcontractor employee who, by reason of previous unsatisfactory performance at the Lincoln Division, VA NWIHCS or for any other rational reason, is considered by the Contracting Officer to be objectionable, as long as ninety (90) days written notification of unsatisfactory performance has been provided to the Contractor, and the problem has not been cured in the ninety (90) day period. Any disagreement about performance still existing at the end of this ninety (90) day period will be handled by the Contracting Officer. PERSONNEL POLICY: The contractor shall be responsible for protecting personnel furnishing services under a resulting contract. To carry out this responsibility, the contractor shall provide the following for these personnel: - Workers compensation - Professional liability insurance - Health examinations - Income tax withholding, and - Social security payments. The parties agree that the contractor, its employees, agents and subcontractors shall not be considered VA employees for any purpose. Contractor s personnel shall be required to dress in a professional manner.
