- This is a sources sought notice for market research purposes only to determine the availability of qualified Service Disabled Veteran Owned Small Business (SDVOSB) sources capable of providing the full range of services described below. Responses should be provided via email to surella.tracy@va.gov no later than 3:00pm central time on December 22, 2017. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. Please clearly identify any information to be considered sensitive or proprietary. Respondents will not be notified of the results of the market research. The NAICS code to be utilized for this acquisition will be 541990 All Other Professional, Scientific & Technical Services Interested capable sources to provide: 1) Company name, address, point of contact, phone number, e-mail address, and DUNS number. 2) SAM registration and/or ORCA as evidence of size status. 3) Brief capability statement (no more than 5 pages) with enough information to clearly demonstrate the capability to provide the full range of required services, and information on past performance providing relevant services. Statement of Work (SOW) Alexandria VA Health Care System 2495 Shreveport Highway Pineville, LA 71360 Introduction/Background The Alexandria Veterans Affairs Medical Center currently has a need for an annual radiation safety assessment uses X ray equipment, MRI, and CT for medical diagnostic measures. To ensure the safety of all personnel and patients, an annual radiation safety assessment is required. 1.0 SCOPE OF WORK The Contractor shall provide all personnel, transportation, equipment and software to provide quality assurance testing survey and certification, recertification when required and written reports in support of the Alexandria Veterans Affairs Health Care System (AVAHCS) and off site clinics. These services shall adhere to the requirements of the National Council on Radiation Protection and Measurements (NCRP), The Joint Commission (TJC), VHA National Health Physics Program (NHPP), the American College of Radiology (ACR), and the Food and Drug Administration (FDA). Task 1: Annual quality assurance testing survey and certification of all diagnostic x-ray and imaging equipment. See Section 5 for Government Owned Equipment List for list of existing equipment. This list may be modified (equipment added or removed) at any time. Task 2: Recertification of imaging equipment prior to first patient use when required due to repairs or modification that may affect the radiation output or image quality. Within 24 hours of contact, the contractor will coordinate with the affected service to have the recertification scheduled. Task 3: Written reports signed by the diagnostic medical physicist documenting the testing, including a summary of methods, instruments used, measurements and deficiencies identified, for all diagnostic x-ray and imaging equipment. This must be completed on new diagnostic x-ray equipment prior to first patient use. Task 4: Deficiencies or non-conformances found during reviews or testing which represent unsafe conditions shall be reported to the Biomedical Department The annual quality assurance testing will include all Radiology, MRI, CT and dental equipment at Alexandria Veteran Affairs Health Care System (AVAHCS) and off site locations including, Pineville, Louisiana, Lafayette, Louisiana, and Lake Charles, Louisiana. All quality assurance testing reports and performance evaluations reports shall be submitted to the AVAHCS Biomedical Department for review and approval after completion of all testing, calibration and surveys of the equipment, facilities and procedures. The reports shall identify all items of noncompliance and include general and specific recommendations for correcting any items of noncompliance. All records (administrative and program specific) created during the period of the contract belong to AVAHCS and must be returned to AVAHCS at the end of the contract. 2.0 QUALITY ASSURANCE TESTING All Quality Assurance Tests shall meet or exceed current NRCP, FDA, NHPP, ACR and TJC requirements and recommendations. All Quality Assurance Tests shall meet or exceed the requirements in the following VHA directives and handbooks and future directives and handbooks. VHA Directive 1105.01, Management of Radioactive Materials VHA Handbook 1105.04, Fluoroscopy Safety VHA Handbook 1105.05, MRI Safety VHA Directive 1129, Radiation Protection for Machine Sources of Ionizing Radiation 3.0 QUALIFICATIONS Prior to commencing work, all contractor employees shall meet certain criteria to perform work under this contract as a Diagnostic Medical Physicist. Qualification documents must be submitted as part of the bid to the Contracting Officer. All qualifications are subject to review by the AVAHCS. 4.0 DIAGNOSTIC MEDICAL PHYSICIST The Diagnostic Medical Physicist who performs the work at CTVHCS shall meet the following education, certification, and qualification standards: Hold a graduate degree in physics, medical physics, biophysics, radiological physics, medical health physics, or a closely related science or engineering discipline from an accredited college or a university. Formal graduate level college coursework in the biological sciences with at least one course in biology or radiation biology and one course in anatomy, physiology, or a similar topic related to the practice of medical physics. Hold current Board Certification by the American Board of Radiology or American Board of Medical Physics. Licensed by the State of Louisiana as a Professional Medical Physicist. Meet all respective NRCP, FDA, NHPP, ACR and TJC requirements for a qualified diagnostic medical physicist.
