Description
NAF General Contract Provisions Statement of Work Individual Service Contract Format Stables Operating Instruction This is a combined synopsis/solicitation for a commercial service. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 1. The solicitation number is FA486118QB012. This solicitation is issued as a Request For Quote (RFQ). 2. This requirement is unrestricted and open to all businesses. The associated NAICS code is 611620, Sports and Recreation Instruction. 3. Below are the contract line item number(s), item description, quantities and units of measure CLIN 0001 - Equestrian Riding Program, 12 months (1st year) CLIN 0002 - Equestrian Riding Program, 12 months (2nd year) CLIN 0003 - Equestrian Riding Program, 12 months (3rd year) Please see Statement of Work for additional details. 4. Description of requirement for the item to be acquired: PERIOD OF PERFORMANCE: TBD 5. Place of delivery is Nellis AFB, Nevada, and inspection and acceptance is FOB destination. To be considered for this requirement, the offer must fully complete the pricing on each CLIN as required. The contractor shall fill out a unit price, the extended price and a total price for all CLINs. Please ensure that all prices, extended prices and total prices are checked and verified for accuracy. DO NOT go two (2) digits beyond the decimal point. To be considered for this requirement, the offeror must provide a technical proposal. The technical proposal should be clear and free of errors. The offeror shall provide sufficient detail to demonstrate a thorough understanding of the requirements and describe an approach demonstrating how the tasks will be accomplished in a timely manner at an acceptable performance level. The Government will award a contract resulting from this solicitation to the responsible offeror whose Acceptable Technical Proposal represents the Best Value to the Government. Best Value will be determined by the proposal deemed as Lowest Priced Technically Acceptable. Offers will be rated by technical acceptability and award will be made to the lowest priced offer. Proposals shall be rated according to the following scale: ACCEPTABLE - Proposal clearly meets the requirements of the solicitation. UNACCEPTABLE - Proposal does not clearly meet the minimum requirements of the solicitation. All proposals will be ranked by total evaluated price. The Government shall then evaluate proposals for technical acceptability in order of their price rankings, lowest to highest. If any of the technical subfactors are rated Unacceptable, the overall rating is Unacceptable and the proposal will not be evaluated further. Tradeoffs are not permitted and no evaluation credit is given to aspects of an offerors proposal that exceed acceptability standards. Submission of Proposals 1. Proposal packages will contain the following information: All proposals shall include the following minimum information: Cage Code, Duns Number, Taxpayer I.D. No., Payment Terms, and/or Discount Terms, Offerors Reps and Certs, Point of Contact, Phone, Fax, Email and Proposed Delivery Schedule. IAW FAR 52.204-7, prospective vendors must be registered in the SAM database prior to contract award. Prospective vendors should visit the SAM website at https://www.sam.gov to register. Lack of SAM registration shall be a determining factor for contract award and vendors who are not SAM registered at time of quote submission shall be considered non-responsive and their quote will not be evaluated. Proposals shall be submitted electronically no later than 22 January 2018, 4:00 PM, PST. Submissions shall be made to the following e-mail address; mary.barrera@us.af.mil and monte.clark@us.af.mil. REAL ID - CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management citing the appropriate paragraphs as applicable. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment. (End of clause) CONTRACTOR ACCESS TO NELLIS AFB, NV REFERENCE: Public Law 109-13, 49 U.S.C., section 30301 and Code of Federal Regulations (CFR) Title 6, Part 37. 1. Effective 15 Aug 2016, the United States Air Force will implement REAL ID Act requirements IAW Public Law 109-13, 49 U.S.C., section 30301 and Code of Federal Regulations (CFR) Title 6, Part 37. 2. This policy change means that after 15 August 2016 contractors seeking base access will not be granted a base pass unless they have an ID card that meets the minimum standards set forth in the regulation listed above. A State ID will no longer be accepted unless it is the new updated "Real ID". A Real ID driver's license or ID card has a gold circle with a star cutout in the upper right-hand corner. 3. Personnel who don't possess REAL ID compliant credentials will have to provide acceptable alternative forms of ID. Some of the more common acceptable credentials are: • United States Passport • Personal Identification Verification (issued by the Federal Government) PIV-I cards (issued by non-Federal Government entities) • U.S Certificate of Naturalization or Certificate of Citizenship (Form N-550) Veterans Identification Card Veterans Health Identification Card • U.S. Permanent Resident Card (Form I-551) Foreign Government issued Passport • DHS issued employment authorization document that contains a photograph (Form I- 766) 4. This will affect current base pass holders when it is time to renew their passes. Current passes will be valid until expiration. 5.
