- Small Business Subcontracting Plan Template Solicitation Provisions & Clauses Organizational Conflict of Interest Certification Solicitation Pricing Worksheet Performance Work Statement's Attachment B Performance Work Statement's Attachment A Performance Work Statement Combined Synopsis Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 - Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a separate written solicitation document will not be issued. This solicitation is being issued under the authority of FAR Part 13.5 Simplified Procedures for Certain Commercial Items. This solicitation is a Request for Quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-96 effective 11/06/2017. The associated North American Industrial Classification System (NAICS) code for this procurement is 541380 with a small business size standard of $15,000,000.00. This acquisition is being procured using full and open competition procedures. Quotes submitted by small and other than small business concerns will be considered. The U.S. Army Public Health Center requires services to analyze between 2,000 and 20,000 drinking water samples from multiple CONUS and OCONUS DoD installations. Carefully review all attached documents including the text of the Combined Synopsis Solicitation, Performance Work Statement (PWS), and provisions & clauses for information on instructions to offerors, evaluation criteria, basis for award, and required submissions.
