Description
Complete description, instructions, provisions & clauses. This combined synopsis/solicitation is expected to result in the award of a firm-fixed-price contract for Carefusion Solutions LLC's Pyxis Point of Use Supply Virtual Client Server and Software Package as described under the requirements section of this solicitation. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. An award, if any, will be made to the responsible Offeror who submits a proposal that: 1. Conforms to the Requirements of the Solicitation. 2. Receives a rating of "Acceptable" on the Technical Capability evaluation factor. 3. Contains the lowest total evaluated price (TEP), provided that the TEP is not unbalanced and is fair and reasonable. Submittal of proposals in response to this solicitation constitutes agreement by the Offeror to all terms & conditions contained herein, which will also be the terms & conditions of any resulting contract. It is the Offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions in full text may be accessed via the Internet website http://farsite.hill.af.mil. The Government reserves the right to award without discussions or make no award depending upon the quality, price fairness, and price reasonableness of the proposals received. The solicitation number for this requirement is FA860118Q0045 and is hereby issued as a Request for Proposal (RFP) using FAR Part 12, Acqustion of Commerical Items and FAR Part 13, Simplified Acquisition Procedures. Produce Service Code: 7010 - Information Technology Equipment System Configuration NAICS Code: 541512 - Computer Systems Design Services Business Size Standard: $27,500,000 Acceptable means of Submission: All submissions must be submitted electronically to the following email address dell.grunenberg@us.af.mil by 17 Jan 2018 at 1:00 P.M. EST. Any correspondence sent via e-mail must contain the subject line " FA860118Q0045: Pyxis Server & Software Package." The entire proposal must be contained in a single e-mail, unless otherwise approved, and must not exceed 5 megabytes, including attachments. E-mails with compressed files will not be accepted. Note that e-mail filters at Wright-Patterson Air Force Base are designed to filter e-mails without subject lines or with suspicious subject lines or contents (i.e.,.exe or.zip files). Therefore, if the specified subject line is not included, the e-mail may not get through the e-mail filters. Also be advised that.zip or.exe files are not allowable attachments and may be deleted by the email filters at Wright-Patterson. If sending attachments with email, ensure only.pdf,.doc,.docx,.xls or.xlsx documents are sent. The email filter may delete any other form of attachments. This solicitation incorporates FAR provisions and clauses in effect through Federal Acquisition Circular (FAC) Fac 2005-96, effective 06 Nov 2017; Defense Federal Acquisition Regulation Supplement (DFARS) Publication Notice (DPN) 20171228, effective 28 Dec 2017; and Air Force Federal Acquisition Regulation Supplement (AFFARS) Air Force Acquisition Circular (AFAC) 2017-1003, effective 03 Oct 2017. DESCRIPTION OF REQUIREMENT Delivery Schedule: 4-8 Weeks After Contract Award Delivery Destination: Medical Materiel Bldg 830, Area A 4881 Sugar Maple Drive Wright Patterson AFB, OH 45433-5529 Delivery Type: FOB Destination (As defined in FAR 2.101-Definitions, the seller or consignor is responsible for the cost of shipping and risk of loss.) Inspection and Acceptance: Both inspection and acceptance will be by the Government at Destination. Requirements: The 88 TH Medical Treatment Facility (MTF) located at Wright-Patterson AFB, OH requires Carefusion Solution LLC's Point of Use Supply Virtual Manager Client Server and Software, specified as follows: •A. Quantity 1 (one) - CareFusion Point of Use Supply Virtual Manager Client Server •1. Part Number: 804 •2. Dimensions: 72"x52"x36" •3. Electrical: 110V Uninterruptible Power Source •4. Must be DIACAP Approved •5. Must interface with current Point of Use system Model 317. •B. Quantity 1 (one) - CareFusion Point of Use Supply Virtual Manager Client Software •1. Part Number: 107-229 •2. Must contain the following software: •a. Supply V9 x Client Wes7 •b. Must contain Windows 7 •C. Delivery: 4-8 weeks after contract award. •D. Installation and Testing: Vendor will be required to install system software and hardware at Wright Patterson Air Force Base, and will perform a successful system test. •E. Warranty: 5 years from date of successful system test. NOTICE: The anticipated award is Firm-Fixed Price-Lowest Price Technically Acceptable (LPTA), and this combined synopsis/solicitation serves a notice of intent to award a sole source contract to Carefusion Solutions, LLC, 3750 Torrey View Court, San Diego, CA 92130, USA. Pursuant to the Federal Acquisition Regulation (FAR) 13.106-1(b)(1)(i), "[c] ontracting officers may solicit from one source if the contracting officer determines that the circumstances of the contract action deem only one source reasonably available." The contracting officer has determined that only one source, Carefusion Solutions LLC, is reasonably available to meet the requirement as no other known source of the required items is DIACAP (DoD Information Assurance Certification and Accreditation Process) approved and currently able to provide items that will fully integrated with the existing point of use infrastructure.
