- This is a Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; proposals are being requested and a written Solicitation is included in this posting. The solicitation number is 36C26318R0245 and is issued as a request for proposal (RFP). This requirement is set aside for Small Business. The associated North American Industrial Classification System (NAICS) code for this procurement is 621999 All Other Miscellaneous Ambulatory Health Care Services with a size standard of $15.0 million. To be considered for award of a Government contract, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. The Department of Veterans Affairs, Network Contracting Office 23 is soliciting proposals for a Fixed-Price Indefinite Delivery/Indefinite Quantity (IDIQ) contract to an offeror capable of providing Turnkey Mobile Lithotripsy services to eligible beneficiaries of the Department of Veterans Affairs Medical Center, Omaha VA Medical Center (VAMC) (hereinafter referred to as VA NWIHCS (Nebraska Western Iowa Health Care System, Omaha Division), located at 4101 Woolworth Avenue, Omaha, NE 68105. The anticipated period of performance will be from October 1, 2018 (or date of award) through September 30, 2019 for the Base Year, with four (4), one year option renewal periods. Option periods are exercised at the sole discretion of the Government. The date for receipt of offers in no later than February 21, 2018, 1:00 p.m. Central Standard Time.
