Description: Page 4 of 6 COMBINED SYNOPSIS/SOLICITATION DENTAL LABORATORY SERVICES FOR THE EAST ORANGE, LYONS AND BRICK MEDICAL CAMPUSES OF THE VA NEW JERSEY HEALTHCARE SYSTEM (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 and 13.5- Simplified Procedures for Certain Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number 36C24218Q0190. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1. (iv) This procurement is being issued as a Service Disabled Veteran Owned Small Business (SDVOSB) set aside. The North American Industry Classification System (NAICS) code is 339116, Dental Laboratories, with a small business size standard of 500 employees. (v) The Contractor shall provide pricing to provide dental lab services for the East Orange, Lyons and Brick Medical Campuses of the VA New Jersey Healthcare System. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will be accomplished by using the prices offered for the last option period to determine the price for a 6-month option period, which will be added to the base and other option years to arrive at the total price. Evaluation of options will not obligate the Government to exercise the option(s). See attached document for pricing tables to be completed and submitted with the quote. The government shall award this procurement to the vendor with the lowest priced quote that is technically acceptable and has acceptable, or neutral, past performance. Vendors with no past performance shall be rated as neutral. PRICING The Offeror shall complete and submit all six tables of the attached Excel spreadsheet titled Schedule of Supplies and Services - Dental Lab Services for pricing evaluation. TECHNICAL The Offeror shall provide a maximum 10 page capability statement that indicates the vendor has the trained personnel, equipment, and track record of working experience in providing removable denture laboratory services. The 10 page limit only applies to this section, information requested below are not included in the page count. The Offeror shall include documentation on the company s Quality Control and Infection Control plans. The Offerors shall indicate in how shipping to and from the VA locations shall be addressed. The Offeror shall provide certifications for all Certified Dental Laboratory Technicians that are assigned laboratory work from New Jersey VA Healthcare System before work commences. If uncertified technicians are being used the vendor shall provide proof of a formal in-house training program run by a certified dental technician. Any subcontractors used in the performance of this purchase order shall be identified and the required certifications and training program information shall be included with the quote. PAST PERFORMANCE The Government shall request the contractor provide a list of contracts completed during the past three (3) years and all contracts and subcontracts currently in process for dental laboratory services. Include the following information for each contract: Name and address of contracting activity Name and telephone number of contract person Dates of contract performance The Government reserves the right to make award without discussions.
