- Notice of Intent for Sole Source This is a synopsis for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes a notice of intent to Sole Source; therefore a written solicitation will not be issued. The U.S. Army Medical Command, Regional Health Contracting Office - Pacific, Bldg. 339 Clark Road, Ft Shafter, HI 96858, intends to award a sole-source firm-fixed price, commercial contract for Preventive Maintenance & Repair Services for Bassett Army Community Hospital (BACH) at Fort Wainwright, Alaska. The scope for this requirement is to procure preventative maintenance and repair services for a Carl Zeiss Vario 700 Microscope, located at BACH. The period of performance will be from approximately 1 April 2018 to 31 March 2019 (1 year) Base Period, with two (2) 1-year Option Periods. Preventive Maintenance Services and repair parts are only available from the licensed original equipment manufacturer (OEM). Due to the proprietary technology, this requirement will be fulfilled using Other Than Full and Open Competition in accordance with FAR 13.106-1(b)(1)(i); and will be sole sourced to Carl Zeiss Meditec, Inc. (CZM), PO Box 8111, Pleasanton, California 94588. The Standard Industrial Classification Code for this procurement is J065, size standard is $20,500,000 and the NAICS code of 811219. This notice of intent is not a solicitation or request for offers and no solicitation package is available. It is not a request for competitive proposals; however, any firm believing it can fulfill the requirement may submit a written response no later than 5 days after the date of publication of this notice, which may be considered by the agency. The written response shall reference Solicitation Number W81K02-18-T-0089 and provide a capability statement (no more than 3 pages) that clearly demonstrates their ability to fulfill the specific requirement identified in this notice. Responses should include the company name, Point of Contact (POC) name, telephone# and email address, company CAGE#, DUNS#, office address, and any brochures which will allow the Government to understand your offerings and options. Capability statements should be submitted electronically to the attention of Tonja D. Jenkins, Contract Specialist, Regional Health Contracting Office - Pacific at tonja.d.jenkins.civ@mail.mil.
