- Government Policy Letter Notice to Offerors - FA520918R0005 Solicitation (SF1449) - FA520918R0005 Exhibit A - E Blank Pricing Schedule Attachment 3 - PP Survey Attachment 2 - PP Ref List Attachment 1 - Performance Work Statement (30 Jan 18) DESCRIPTION: The Air Force Medical Service (AFMS) has a requirement for five (5) Registered Nurse (RN) Case Managers, two (2) Licensed Practical Nurse or Licensed Vocational Nurse (LPN/LVN) Utilization Managers, one (1) Nurse Practitioner (NP) DRHA provider and one (1) RN, Family Practice Clinical Nurse at Air Force medical treatment facilities in Japan. The contractor shall provide all labor, materials, tools, equipment, transportation, and supervision necessary to recruit, qualify, place, manage and retain the following case manager, utilization management, and nurse professionals: Kadena AB* Two (2) Registered Nurse Case Managers One (1) LPN/LVN Utilization Manager * Personnel are also required to work at United States Naval Hospital Okinawa Misawa AB One (1) Registered Nurse Case Manager Yokota AB Two (2) Registered Nurse Case Managers One (1) LPN/LVN Utilization Manager One (1) Nurse Practitioner (NP) DRHA Provider One (1) Family Practice Clinical Nurse (RN) PRE-PROPOSAL CONFERENCE: A pre-proposal conference will be conducted at the Contracting Squadron, Bldg. 620 (Conference Room 109), Yokota Air Base on 27 February 2018 at 0900 hours (Japan-Standard Time) for the purpose of answering questions regarding this solicitation. Submit names of all attendees (not to exceed 3 persons) to Mr. Yuki Watabe by e-mail to yuki.watabe.jp@us.af.mil or fax at 042-530-3319 at least two work days prior to the conference or to Ms. Namiko Kaneko at namiko.kaneko.1.jp@us.af.mil. The information must be provided in advance for ensuring your access to the Yokota Air Base and attendance at the conference. In case a telephone conference is preferred due to location issue, attendance request email must include the available phone number(s). Award will be made to the offeror whose offer represents the best value to the U.S. Government considering the evaluation factors stated in the solicitation posted on this web site.
