Description

This is a Combined Synopsis/Solicitation Request for Proposal (RFQ) to provide pharmacy services, specifically filling an estimated 16,906 (3024 Macon 4512-Albany, 2586-Tifton, 3484-Dublin, 1800-Milledgeville, 1500-Perry) prescriptions on an annual basis, to be located within a ten (10) mile radius or adjacent to the premises of the Department of Veterans Affairs Community Based Outpatient Clinic (CBOC) for Macon is located at 5398 Thomaston Road, Suite B, Macon, GA 31220, for the Community Based Outpatient Clinic (CBOC) for Albany is located at 417 4th Avenue, Albany, GA 31701 for the Community Based Outpatient Clinic (CBOC) for Tifton is located at 1824 N. Ridge Avenue, Tifton, GA 31794, for the main Dublin VAMC campus located at 1826 Veterans Blvd, Dublin, GA 31021, for the Milledgeville CBOC located at 2249 Vinson Hwy SE, Milledgeville, GA 31061 and for the Perry CBOC located at 2370 S Houston Lake Road, Kathleen, GA 31047. This requirement is a 100% Service Disabled Veteran Owned Small Business set aside in accordance with Public Law 109-461. The NAICS code for this requirement is 325412 and the Small Business Size Standard of $1250 Employees. Eligible and interested vendors should download a copy of the RFQ Solicitation document 36C24718Q0234_1, which contains the SOW and all corresponding attachments, from the FBO website (www.fbo.gov). The solicitation has instructions on how to submit quotes in accordance with FAR 52.212-1. Please check the posting daily for updates. The government will not be contacting interested contractors regarding amendments. Please note the due date established by the solicitation.
