Page 3 of 3 SOURCES SOUGHT NOTICE The Department of Veterans Affairs, Network Contracting Office (NCO) 15 is conducting market research to help determine the availability and technical capabilities of qualified vendors who can deliver Unit Dose Radiopharmaceuticals to the to the Marion IL VAMC at 2401 West Main Street, Marion, IL. and are within the 60-mile radius of this location. Due to the critical need for these supplies in a timely manner, only vendors within a 60-mile radius of the Marion IL VAMC need to respond to this which is for Base with four Option years Sources Sought. Contractor agrees to provide radiopharmaceutical preparation of doses, tagging of blood, and quality testing shall be performed at contractor facility by radiopharmacist licensed in pharmacy by the State Board of Pharmacy and who meets equivalent qualification in nuclear pharmacy as described by the NRC in CFR 35 and delivered to the Marion IL VAMC at 2401 West Main Street, Marion, IL based on this Statement of Work. Contractor shall provide services and staff, and otherwise do all things necessary for or incidental to the performance of work, as set forth below: Must be delivered directly to the Nuclear Medicine Service at Marion IL VAMC daily quantity of radiopharmaceuticals by 6:30 a.m., as requested from the previous day. Deliver directly to the Nuclear Medicine Service at Marion IL VAMC quantity of radiopharmaceutical requested on an as needed/emergent basis to arrive within three (3) hours of notification. Other delivery times may be outlined by individual the VAMC under the awarded contract. Contractor s employees will be provided access to the Hot Lab for deliveries and pick-ups of radiopharmaceuticals and blood specimen for Induim-111 Oxine White Blood Cell tagging by an authorized VA staff member or the Nuclear Medicine Technologist. Delivery personnel will be bonded by the contractor. Shipments called when needed may be requested during regular work hours, or after, as emergency, including weekends, holidays and nights. Shipments requested as an Emergency will be delivered as follows: Requests placed after regular work hours (weekend, holiday, and night) will be delivered within three (3) hours, or as specified. All shipments will be labeled as Radioactive Materials (RAM) in compliance with all pertinent regulations. All Radiopharmaceuticals, Radioassay Kits and/or Analytic Kits are to be furnished by the Contractor in strict accordance with the terms and conditions of the contract and all applicable Local, State and Federal Laws. Delivery dates, times, frequency of delivery, specific dosages, and all other specific requirements by Marion IL VAMC under this contract, will be strictly adhered to. All shipments will have a Packing Slip enclosed, giving information about the product and its identification. Contractor shall provide bar coded inventory list of products and previous daily quality control sheet. Plus (for billing purposes) the VA Medical Center s Purchase Order Number issued for the current fiscal quarter or month, and possible the Contractor s invoice number. (Incorrect P.O. numbers may cause a substantial delay in payments.) The C&A requirements do not apply. Security Accreditation Package is not required. Contractor shall not be responsible for safeguarding and maintaining patient information due to the non-use of patient information in the procurement of the radiopharmaceuticals. All patient doses are assigned an inventory number which will coincide with the patient information known only to the nuclear medicine staff. List of some items may be required from Vendor once contract awarded RADIOPHARMACEUTICAL Est. YES or NO This notice is to assist the NCO 15 in determining sources only. This announcement is not a request for proposals or quotations. The Government is not committed to award a contract pursuant to this announcement. The Government will not pay for any costs incurred in the preparation or submission of information in response to this announcement.
