- Atch 7 - Instructions to Quoters Atch 6 - Evaluation Criteria Atch 5 - Background Questionnaire Atch 4 - Memorandum of Understanding Atch 3 - Clauses and Provisions Atch 2 - Bid Schedule Atch 1 - Performance Work Statement Solicitation Number: FA4887-18-Q-JB-01 - CATHOLIC MUSICIAN This is a COMBINED SYNOPSIS/SOLICITATION for commercial services prepared in accordance with the format in FAR subpart 12.6 pursuant to FAR 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Purchase Request Number is: F2U0308009A005 Submit written offers IAW CLIN structure outlined in announcement. Oral offers will not be accepted. All potential vendors responding must be registered with System for Award Management (SAM) at https://www.sam.gov. This solicitation document and incorporated provisions and clauses are reference to Federal Acquisition Circular (FAC) 2005-83 dated 02 July 2015. The North American Industrial Classification Standard (NAICS) is 813110 - Religious Organizations. Size Standard is $7,500,000.00 Request For Quote (RFQ) for: CATHOLIC MUSICIAN The 56th Contracting Squadron, Luke AFB AZ intends to award a Firm Fixed Priced contract. All services are to be performed in strict accordance with the attached Performance Work Statement. This requirement will be a small business set-aside. Period of Performance is for Base plus 3 Option Periods as follows: BASE: 22 March 2018 - 30 September 2018 Option Year One: 01 October 2018 - 30 September 2019 Option Year Two: 01 October 2019 - 30 September 2020 Option Year Three: 01 October 2020 - 30 September 2021 CLIN structure is as follows: 0001 Catholic Musician 1001 Catholic Musician 2001 Catholic Musician 3001 Catholic Musician The clauses and provisions applicable to this acquisition are listed on Attachment 3. Additional clauses may also be added at the discretion of the contracting officer upon award. Site Visit: There will be no site visit. RESPONSE DATE: All questions regarding the solicitation must be submitted in writing to the points of contact listed in solicitation no later than 2:00 P.M. Mountain Standard Time on 6 February 2018. Proposals are due by 2:00 P.M. Mountain Standard Time on 21 February 2018.
