COMBINED SYNOPSIS/SOLICITATION NOTICE 36C26218Q0310 Responses are due by 12:00 pm (NOON, PST), Thursday, February 08, 2018 COMBINED SYNOPSIS/SOLICITATION NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-87, dated March 07, 2016. Solicitation 36C26218Q0310 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued as Veteran Owned Small Business Set-Aside. NAICS Code is 339113 Surgical Appliance and Supplies Manufacturing and the Small Business Administration s (SBA) size standard 750 Employees. The Government is seeking a vendor who can provide the following items for the VA Long Beach Healthcare System: Item Number Description QTY Unit Unit Cost Total Price DV64D-HH-S INTELLIPAP 2 AUTOADJUSTW/HEATED HUMIDIFIER & MODEM W/5 YEAR CELL SERVICE WARRANTY 320 EA   This solicitation is for brand name (exact match) only and offerors shall be an authorized distributor by the manufacture and must be able to offer full manufacture warranties, including full VA equipment compatibility, at a fair and reasonable price offering best value to the Government. OFFEROR MUST BE ABLE TO DELIVER ALL LISTED ITEMS FOR THE LONG BEACH VA HEALTHCARE SYSTEM WITHIN THE DELIVERY SCHEDULE BELOW: Delivery Schedule: 40 UNITS PER WEEK FOR 8 WEEKS OR AS INSTRUCTED Place of Performance: VA Long Beach Healthcare System, 5901 East 7th Street, Long Beach, CA 90822.
