Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W91YTZ-18-T-0161 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 and Defense Federal Acquisition Supplement Publication (DPN) 20161222. This acquisition is issued on a 100% Small Business Set-Aside basis under NAICS code 323111 and size standard is 500 employees. All responsible sources may submit a quotation which shall be considered by the agency. The purpose of this solicitation is to procure blood collection bag labels supplies and services for Sullivan Memorial Blood Center at Martin Army Community Hospital, Fort Benning, Georgia. Period of Performance: Date of Award through 30 September 2018, subject to 52.232-18 -- Availability of Funds. Quotes are due by 22 February 2018 at 5:00 pm EST. Have labels printed in compliance with the Uniform Labeling of Blood and Blood Components and feature adhesive that is compositionally compliant with FDA 21 CFR 175.105. 4. Have labels pre-printed and specifically designed for the labeling of blood, blood components and blood segments. 5. Provide all requested information in Addendum 52.212-1 Prices will be reviewed to determine reasonableness based on market research. The Government will issue an order resulting from this RFQ to the responsible contractor whose quote, conforming to the RFQ, is Lowest Priced, Technically Acceptable (LPTA). Award will be on an all or none basis.
