This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, with a small business size standard of 1000 Employees. The Veterans Health Care System of the Ozarks located in Fayetteville, Arkansas is seeking to purchase MicroMedical brand name or equal, Reclining Rotational Vestibular Chairs for its Springfield, Missouri Community Based Outpatient Clinic. All interested companies shall provide quotations for the following: Supplies Line Item Description Quantity Unit Price Total Price 0001 MICROMEDICAL SYSTEM 2000 RECLINING ROTATIONAL VESTIBULAR CHAIR WITH VNG S2000RSYSTEM INCLUDES: SPECTRUM SOFTWARE PART # S2000RSYSTEM 1 EA 0002 MICROMEDICAL AIR FX CALORIC IRRIGATOR - PART #8503836 1 EA 0003 INSTALL/TRAINING PART #8074 1 EA Delivery shall be provided no later than June 5, 2018. Delivery terms are FOB. The contractor shall deliver line items 1, 2, and 3 to: Veterans Health Care System of the Ozarks Springfield Community Based Outpatient Clinic 1850 W. Republic St. Springfield, MO 65810 Award shall be made to the offeror whose quotation offers the best value to the government, considering technical capability, past performance, and price. The government will evaluate information based on the following evaluation criteria: (1) technical capability factor "meeting or exceeding the requirement, (2) past performance, and (3) price.     The full text of FAR provisions or clauses may be accessed electronically at http://acquisition.gov/comp/far/index.html. The following solicitation provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items (JAN 2017) ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS COMMERCIAL ITEMS Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The following provisions are incorporated into 52.212-1 as an addendum to this solicitation: 52.212-2, Evaluation - Commercial Items:   The Government will award a Firm-Fixed Price contract resulting from this solicitation to the responsible contractor whose quotes conforming to the solicitation will be most advantageous to the Government. Award will be made to the lowest priced quote that meets or exceeds ALL requirements of this solicitation. The Content of the Non Price Factors/Technical Part detailed below must also be included with the package. Prospective Contractors are required to submit two separate documents; one (1) Part A Technical Part and one (1) Part B Price Part IAW Schedule list above. If both Parts are not received, the quote will be considered non-responsive, and will not be considered for award. Quotes must be electronically submitted on or before the Solicitation close date and time 10:00 AM CST on March 14, 2018. No faxed or hand-delivered quotes will be accepted. Submit quotes to arlene.blade@va.gov. Part A - Content of the Non Price Factors/Technical Part will be evaluated on Pass / Fail basis -Ability to meet the technical/performance specifications of the Item Description and quantity Item quoted must meet at the minimum the salient characteristics of item described within the Schedule above. Offeror to provide technical brochures to support functionality features of the items required.   In addition, Electronic data or virtual link in order to examine capabilities shall be provided.  Offerors shall provide in detail for each specification that is required that the particular specification is or is not offered with a reference to the quote/brochure/data provided that the specification required is in fact offered.   If offering a product that specified herein, the vendors are required to provide descriptive literature in sufficient detail to determine if the items quoted are technically acceptable. Offeror shall also provide evidence of being an Authorized Reseller (Distributor) for product offered.   This can be substantiated with a link to Manufacturer s web site that supports offeror s distributorship or other proof from the manufacturer. The attached document titled S15 Technical Verification Sheet is required to be submitted with the quote. Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.   All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. New Equipment ONLY: No remanufactured or Gray Market items: Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers.   No remanufactures or gray market items will be acceptable. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract.   Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.   If contractor fails to provide sufficient Part A information or the information has not passed the evaluation, the quote will be deemed not acceptable and will not be further evaluated. Part B - Content of the Price part Price schedule Total price for CLINs as described in Schedule above. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation. (b) To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications. (c) The Contracting Officer will evaluate "equal" products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the solicitation. All insert "quoters, offerors, or bidders"] shall submit the following: [insert number of copies and information to be submitted]. This is an open-market combined synopsis/solicitation for products as defined herein.    The government intends to award a purchase order as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quoter shall list exception(s) and rationale for the exception(s).
