- Pre-Solicitation Notice - continued VA CMOP National Contracting Office RFQ 36C77018Q0348 BPA for Patient Centric Labels (PC Labels) Patient Centric labels are applied to Rx bottles and as overlays on pre-package non-prescription pharmaceutical items being packaged and sent to Veterans. See CMOP 2012 Youtube video for further reference on the CMOP Mission and Rx Labels https://www.youtube.com/watch?v=GfYHffF7Qhs BPA for Patient Centric Labels at CMOP Facilities: Lancaster Tx (763) Murfreesboro TN (764) Leavenworth KS (760) RFQ to be posted no later than Friday 9Mar18 Sample Labels will be a requirement from all Bids on this RFQ Sample Label testing by the CMOP Facilities will be part of the selection criteria 3 Rolls 1000 labels per roll 1 roll for each CMOP will be mailed to the CO at vendors expense Label Specs are provided below for vendor to reference when preparing Samples for Testing. Actual in use - Patient Centric Labels are available from the CO by email request. Interested Vendors in their email request must provide mailing address and POC. CO will UPS to requesting Vendors 5 actual in use Patient Centric Labels for their reference. Sample Labels (3 rolls) due in this office no later than 28 Mar 18.
