- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Combined Synopsis "Request For Quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular [FAC 2005-96, effective 11/06/17]. The associated North American Industrial Classification System (NAICS) code for this procurement is [621399] Offices of All Other Miscellaneous Health Practitioners, with a small business size standard of [$7.5 Million]. The Department of Veterans Affairs, Palo Alto VA Health Care System, 3801 Miranda Avenue Palo Alto, CA 94304 requires group/practice to provide services necessary to perform onsite per-procedure Perfusion Services to eligible beneficiaries of the Department of Veterans Affairs Medical Center, VA Palo Alto Health Care System. Period of Performance includes a 1-year base period with 1 / (6) month option period if the requirement continues to exist.
