- Subject: Wellness Center Support Staffing and Nurse Executive Services This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 140D0418Q0170 and is a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97 / 01-24-2018. This is a full and open competition. The NAICS is 621999 All Other Miscellaneous Ambulatory Health Care Services with a size standard of $15 Million. Description of requirements for the items to be acquired: Staff Nurse and Exectuve Nurse requirements as depicted in the attached Performance Work Statement (Attachment Terms and Conditions) Date(s) and place(s) of delivery and acceptance and FOB point: Base year will be twelve months, with four twelve month option periods. The Office of Facilities and Administrative Services is located at 1849 C Street, NW, Washington, DC. The following provisions apply: FAR 52.212-1, Instructions to Offerors ¿ Commercial with the addenda found in Attachment 1 - Instructions to offerors. FAR 52.212-2 applies to this acquisition and can be found in Attachment 1 ¿ Instructions to offerors.
