VHAPM Part 813.106 Page 1 of 1 Original Date: 08/30/17 DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 for Over Micro-Purchase Threshold but Not Exceeding the SAT ($150K) Acquisition Plan Action ID: VA250-18-AP-4340 Contracting Activity: Network Contracting Office (NCO) 10, 260 E. University Ave., Cincinnati, OH 45219 Agency: Department of Veterans Affairs, VISN 10, VA Medical Center, 10701 East Boulevard, Cleveland, OH 44106. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: Request is for a visual field analyzer to test for glaucoma, neurological disease, macular disease, and others. The equipment is necessary to maintain clinic flow and reduce in-office wait times for veterans. Several clinics have expanded and this has increased the demand for visual field testing. Updated interface provides less risk for user/interface error, lens technology reduces risk of test/results error, kinetic perimetry provides increased testing area, DICOM inclusion allows for direct upload of results to records to better meet privacy standards, also compatible with FORUM. The Humphrey units are the true gold standard of care and are better able to evaluate for and monitor disease progression. It is utilized at all VA, DoD, and IHS Eye Clinics. It is the standard of care for: diagnosing and managing all types of optic nerve disorders; monitoring multiple medications for retinal toxicity; determining stage of visual impairments, including legal blindness. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: This is a direct replacement of outdated equipment. Brand name only is being requested to interface with legacy data. Market research indicates other manufacturer products are not compatible with the agency s needs. Description of market research conducted and results or statement why it was not conducted: VetBiz was queried for both SDVOSB & VOSB concerns. The Government believes Corps Biologics, LLC is the only authorized Service Disabled Veteran Owned Small Business (SDVOSB) distributor of the requested equipment. Per a memo received from the manufacturer, Corps Biologics, LLC is the sole SDVOSB authorized reseller. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
