- SOW Word Combined Synopsis/Solicitation for Locum Physician Services 1. This is a combined sysopisis/solicitation for commercial items and services prepared in accordance with FAR 5.101 and 5.201. This announcement constitutes the only solicitation and the RFQ1269232 was submitted at eBuy through GSA Advantage. 2. The NAICS code is 621111-Offices of Physicians (Except Mental Health). Offerors must be registered in the System for Award Management (SAM) at www.sam.gov, in order to be considered for award. 3. FAR Clauses 5.101, 5.201, 52.212-1 through 52.212-5 apply to this procurement. For the exact text and wording of clauses and provisions please see http://farsite.hil.af.mil. The Government contemplates an award of a Firm Fixed Price purchase order contract resulting from this solicitation. 3. Description of services: Contact Coleen Reed, for attachments of the Scope of Work; and four other documents that go with this SOW. 4. The Government will award a single purchase order contract resulting this RFQ to the responsible offeror whose offer represents the best value in terms of technical capability and price. All non-price factors (i.e., technical capability and past performance) are significantly more importand than price. 5. Offerors must be registered at www.sam.gov in System for Award Management. 6. Offerors tor must submit a response to this RFQ due March 15, 2018 5:00 PM PST. All questions or inquiries must be submitted to the contract officer via electronic mail (email) no later than March 13, 2018.
