Description

This Sources Sought/Market Survey Notice is being conducted by Network 10 Contracting, PO Box 492, Ann Arbor, MI 48106 to identify other sources that are capable of providing residential treatment, Safe Haven Model Housing Program for chronically homeless male veterans, beneficiaries of the John D. Dingell VAMC, 4646 John R. Street, Detroit, MI 48201. The contractor shall provide intensive case management services utilizing the Recovery Model to target hard to reach, chronically homeless veterans with mental illness and substance abuse disorders utilizing onsite staff 24/7. Facility to be located in Macomb, Oakland or Wayne counties, State of Michigan, USA. Period of Performance: five (5) one-year ordering periods. Type of contract, Indefinite Delivery, Indefinite Quantity, up to 11 beds, minimum of one bed day, maximum of 20,075 bed days.
