- Description of Requirements; Terms This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this requirement is FA860118Q0071 and is hereby issued as a Request for Proposal (RFP) using FAR Part 12, Acqustion of Commerical Items and FAR Part 13, Simplified Acquisition Procedures. Background : The 88TH Medical Treatment Facility (MTF) located at Wright-Patterson AFB, OH requires an FDA-approved Posterior Sacroiliac Fusion (PSIF) Kit for a Sacroiliac Joint Fusion Procedure scheduled on 21 MAR 2018, as described in the attached file. A PSIF is required for patients that have complete SI joint disruption, transiliac or transsacral fracture-dislocation of the SI joint with an insignificant, small fragment. In most cases the patient becomes infected and the patient has to be brought in for surgery to see how damaged the bone is and if stabilization of the completely disrupted sacroiliac (SI) joint can be implemented. This combined synopsis/solicitation is expected to result in the award of a firm-fixed-price contract for one (1) Posterior Sacroiliac Fusion (PSIF) Implant Kit as described below and in the requirements section of the attached file. 1. Posterior implants and accessories. Lateral implants are not acceptable for this type of procedure. 2. Quantity 2: 8x10x25mm PSIF Implants 3. Quantity 2: Guide Wire 4. Quantity 1: 7mm Cannulated Drill 5. Quantity 1: Bone Marrow Aspiration Needle 6. 1cc DBLogix DBM Putty • All Itesm 1-6, or equivalents, are required. • The single implants should consist of allograft bone to allow for easy posterior insertion. • Date Items Required: 21 MAR 2018 (Carried in by the vendor) • Date of Surgery: 21 MAR 2018 The implants will be implanted into the patient and will be installed by the surgeon via open incision. A vendor representative shall be present during the procedure to provide guidance, and alternative items as is customary for this type of procedure.
