Description
SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Coatesville VA Medical Center, 1400 Blackhorse Hill Road, Coatesville, PA 19320, is conducting a market survey to identify potential sources to provide Ambulance and Wheelchair Van Transportation Services to its veteran beneficiaries. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. The purpose of this market research is to identify capable sources that are both experienced and qualified to provide Ambulance and Wheelchair Van Transportation Services, including all vehicles, personnel, management, supplies, transportation, equipment and reports necessary to cover ground ambulance and wheelchair transport service. Contractor care shall cover the full range of paramedic and ambulance services provided in the local community to include the main Coatesville VA Medical Center and two Community Based Outreach Clinics located in Pennsylvania. Paramedics responding to codes will bring equipment/supplies required by state regulations, and Advanced Life Support (ALS) equipment and medication to include, but not limited to, cardiac monitoring, intubation equipment, automated external defibrillator (AED), video laryngoscope, pulse oximetry, glucometer, portable oxygen, protective clothing and written procedures. The Contractor shall have available the appropriate number of vehicles to provide services in a timely manner as outlined by the contract. The North American Industry Classification System Code (NAICS) is 621910 and the small business size standard is $15 Million for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years or whether you have a socio-economic designation. Capabilities Statement and Documentation: Firms/Companies are invited to provide their capabilities, experience and knowledge in providing Ambulance Transportation Services. Sources are asked to provide information to demonstrate their answers to the following items: Company name, address, website, DUNS number, business type, socio-economic status (e.g., SDVOSB, Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), general small business, large etc.), and person of contact (including telephone number & email address). Indicate and explain your capability of providing this service including maintaining any required security clearances and or additional licensing. Include the states that you are licensed to provide ambulance services in. Provide a summary of the type transportation equipment that will be used and any relevant licensing. Please indicate your company's size under NAICS Code 621910. Please indicate any government contracts held including government agency, contract number, dollar value, period of performance, and government contact information. In addition to a Capability Statement which includes the above information, submissions can include specifications, brochures, and manuals as attachments. Information regarding your firm's capacity and capability to provide such services are to be submitted electronically on or before Monday, 03/26/2018 prior to 12:00 pm (EST) addressed to Cynthia Grygier, Contracting Officer: cynthia.grygier@va.gov.
