REQUEST FOR INFORMATION: 36C26218Q9219 VA LOMA LINDA THIS REQUEST FOR INFORMATION (RFI) IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). This is NOT a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Businesses (VOSB) relative to NACIS 541990 sources (size standard 1000 Employees). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought synopsis, a solicitation announcement may be published in the FedBizOpps or GSA eBuy websites. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The Department of Veterans Affairs, Network Contracting Office 22, is seeking sources to provide the services listed below: SERVICES SOUGHT: Provide spinal cord neurophysiological monitoring services with remote neurologist physician interpretation by certified neurological physician. Vendor shall provide all equipment, labor, materials, parts, supplies, and supervision required to provide certified neurophysiological monitoring for spine surgery at VA Loma Linda Healthcare System (VALLHCS). Vendor shall provide on-call and scheduled services at VALLHCS. Vendor will provide the technician and equipment to perform EEG surgical monitoring, brainstem surgical monitoring, somatosensory spinal cord monitoring, EMG, and pedicle stimulation Transcutaneous, Motor Evoked Potential (TcMET) where appropriate. Live remote monitoring by a certified neurologist will be performed during the relevant portions of the procedure. Vendor will be responsible for maintaining personal equipment and ensuring needed supplies are available for each use. Vendor will be available for services on both a regular business hour basis (Monday Friday) and for emergency services which may include weekends, nights, holidays, etc. If you are interested, and capable of providing the required services please provide the requested information as indicated below. Responses to this notice should include: company name, address, point of contact, e-mail and phone number, and size of business pursuant to the following questions: (1) Please indicate the size status and representations of your business, such as but not limited to: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Small Business, HUBZone, Woman Owned Small Business (WOSB), Large Business, etc.)? (2) Is your company considered small under the NAICS code identified under this RFI? (3) Does your company have an FSS contract with GSA or the NAC or are you a contract holder with NASA SEWP or any other federal contract? If so, please provide the contract type and number. (4) If you are an FSS GSA/NAC or NASA SEWP contract holder or other federal contract holder, are the referenced items/solutions available on your schedule/contract? (5) Please provide general pricing for your services for market research purposes. (6) Please submit your capabilities with regards to the services required and any information that can assist the Government (VA) in making acquisition decisions. Please include current contracts, licenses, certifications, etc.. Responses to this notice shall be submitted via email to cecil.nichols@va.gov Telephone responses shall not be accepted. Responses must be received no later than Thursday, 03/15/2018 11:00AM PST. If a solicitation is issued it shall be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this request for information. Prospective bidders must be authorized re-sellers in order to participate in the solicitation process.
