- outside lights lights This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Navajo Area Indian Health Services, Pinon Health Center (PHC), Facility Department is requesting for submittal of offers for a contractor to provide outdoor lights. Per FAR 6.302-1(c)(2) PHC has identified a specific brand name, but will accept brand-name or equal descriptions. Pinon Health Center (PHC) provides medical care for approximately 12,000 Navajo. Serving a rural area, many of our patients travel 100 miles round trip to receive care. The Pinon Health Center is an Ambulatory Care outpatient facility. Services offered are: Outpatient Primary Care, Podiatry, Women's Health, Mental Health, Pharmacy, Optometry, Dental, PT, OT, Speech Pathology, Audiology, Lab, Public Health and School Health. EVALUATION CRITERIA PHC reserves the right to issue a single award or multiple awards to the contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following Evaluation Factors will be considered: Past Performance, Logistics, and Warranty. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. The socio-economic status of an offeror may also be considered, should one or more quotes represent the best value. SUBMITTAL PROCESS Email your offer to the Purchasing Agent for this action is: Edith Denny, Purchasing Agent, 928-725-9801, edith.denny@ihs.gov. Questions may be submitted in writing via email prior to RFQ Closing Date. To be considered for award, your offer shall include the following documents/information: 1. Provide Price Quotation. The Government will consider competitive rates upon negotiation. 2. Provide Product Specifications. 3. Provide evidence in experience in providing equipment of similar scope and complexity to Federal Government or other large commercial/government entities within the last three (3) years of operation. 4. Provide evidence that your offer meets the attached Specifications.
