- Past Performance Questionnaire - Fillable Cost-Price Template - Fillable Amendment Update The purpose of this update is to clarify the plug number under Section B Items and to provide fillable forms for the "Pricing Template" and the "Past Performance Questionnaire". 1. Offerors are to utilize the plug number of $5,000 as their price for Optional Items 1001, 2001, 3001, and 4001, Curriculum Update. The requirements for these Items are not yet known, hence the use of a plug number to be fair to all vendors providing a quotation. After the award, when the requirements for the Curriculum Updates become known, MCC will negotiate a Firm Fixed Price for these items. 2. A fillable "Pricing Template" and "Past Performance Questionnaire" are provided as Attachments. The quotation due date remains unchanged at March 22, 2018 at 2:00pm Eastern. ===================================================================================== Amendment 0001 95332418Q00230001 The purpose of this amendment is to provide answers to vendor questions (see attached amendment). The due date for offers remains unchanged at 03/22/2018 2:00pm EST. Constraints Analysis (CA) Curriculum Development and Training for Department of Policy and Evaluation (DPE) Economic Analysis and Monitoring and Evaluation Divisions Request for Quotation 95332418Q0023 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 and Subpart 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. a) The solicitation number is 95332418Q0023. This solicitation is issued as a Request for Quotation (RFQ). b) This solicitation is not a set-aside. c) Contract line item number(s) and items, quantities and units of measure are included in the solicitation. d) The Millennium Challenge Corporation (MCC) seeks offers for a Firm Fixed Price (FFP) type contract for Constraints Analysis Curriculum Development and Training for the MCC Department of Policy and Evaluation (DPE), Economic Analysis and Monitoring and Evaluation Divisions. MCC intends to award a single Firm Fixed Price (FFP) type contract with a one-year base period and four one-year option periods. e) Dates and places of delivery are included in the solicitation document. Acceptance is at the Government location and all deliveries are FOB destination. f) The provision at 52.212-1, Instructions to Offerors -- Commercial applies to this acquisition. Addenda are included in the solicitation. g) The provision at 52.212-2, Evaluation -- Commercial Items applies to this acquisition. The following evaluation criteria are hereby included in paragraph (a) of the provision for the Blanket Purchase Agreement: A. Technical Capability a. Sub-Factor 1: Technical Approach b. Sub-Factor 2: Staffing B. Past Performance C. Price h) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with their offer. See Section K of the solicitation. i) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items/Alternate I, applies to this acquisition. Addenda are included in the solicitation. j) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. See Section I for the additional FAR clauses that are applicable to this acquisition. k) The contracting officer has determined that additional terms and conditions (MCC-specific clauses) are necessary for this acquisition.
