Draft Performance Work Statement Sources Sought Form THIS IS A SOURCES SOUGHT NOTICE AND SHOULD NOT BE CONSTRUED AS A SOLICITATION ANNOUNCEMENT. THE SUBMISSION OF THIS INFORMATION IS FOR PLANNING PURPOSES ONLY. IT IS NOT TO BE CONSTRUED AS A COMMITMENT BY THE GOVERNMENT TO PROCURE ANY SERVICES, NOR IS IT THE INTENT OF THE NEBRASKA ARMY NATIONAL GUARD TO AWARD A CONTRACT ON THE BASIS OF THIS SOURCES SOUGHT REQUEST OR OTHERWISE PAY FOR THE INFORMATION SOUGHT. 1. INFORMATION: This Sources Sought Notice is a market research tool being used to determine the availability and adequacy of potential small business sources prior to determining the method of acquisition. The information provided in response to this notice will assist the Government in determining the extent of the small business and socio-economic market. The primary type of work will include medical and dental non-clinical case management and administrative assistant/care coordinator staffing in support of Individual Medical Readiness (IMR) for the Nebraska Army National Guard. Any resulting solicitation will be issued under the North American Industry Classification System (NAICS) code 561110 and size standard is $7.5 million. The Army National Guard anticipates an acquisition for a base and four option years. 2. PROJECT SCOPE: This is a possible future requirement for Non-Clinical Medical Case Managers and Administrative Care Coordinators in support of Individual Medical Readiness (IMR). The Case Manager position shall assess and coordinate treatment for Soldiers with conditions that have or will result in them being medically or dentally non-deployable. The Case Manager shall maintain a minimum appropriate licensure and credentials (current Registered Nurse (RN) License with Basic Life Support (BLS)). The Administrative Care Coordinator position shall assist the Case Managers with their workload. A draft Performance Work Statement (PWS) is attached. This PWS is a draft. The Government retains the right to adjust/alter/change it at any time. 3. SUBMISSION REQUIREMENTS: all interested firms shall complete the Sources Sought Information Request Form and submit it via email to Jarod.w.epp.mil@mail.mil. Submissions shall be received by 10:00 AM, Central Standard Time, 28 March 2018 and shall not exceed four (4) one-sided 8.5x11 pages, to include Request Form. This synopsis is only for market research purposes. This Sources Sought does not constitute a request for proposal nor does it restrict the Government as to the ultimate acquisition approach. The Government shall not reimburse respondents for any costs incurred in preparation of response to this notice. All data received in response to this Sources Sought marked or designated as corporate or proprietary will be fully protected from release outside of the U.S. Government. No hard-copy or facsimile submissions will be accepted. Interested offeror's shall address this requirement through this Sources Sought in written format via email to Jarod.w.epp.mil@mail.mil. Calls and fax transmittals in response to this Sources Sought will not be accepted. Personal visits for discussing this announcement will not be scheduled.
