Description
FULL ADDENDUM INCLUDING STATEMENT OF WORK AND PROVISIONS/CLAUSES (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This solicitation, 12639518Q0086, is issued as a request for quotation (RFQ) in accordance with FAR Part 13. (iii) The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulation (FAR) can be accessed on the Internet at http://farsite.hill.af.mil/ or at https://www.acquisition.gov/. (iv) This RFQ is set aside for small business. The NAICS code is 325320 and the small business size standard is 1,000 employees. (v) This requirement is the United States Department of Agriculture (USDA), Animal Plant Health Inspection Services (APHIS), Veterinary Services (VS) to purchase the following items in the following quantities: CLIN Description Est Qty Dispenser Capability in Days Unit Price Extended Price 001 Base Ordering Period: Award - 3/31/2019 100,000 $ $ 002 Option Period 1: 4/1/2019 - 3/31/2020 100,000 $ $ 003 Option Period 2: 4/1/2020- 3/31/2021 100,000 $ $ 004 Option Period 3: 4/1/2021-3/31/2022 100,000 $ $ 005 Option Period 4: 4/1/2022- 3/31/2023 100,000 $ $ * Price shall include all costs needed to accomplish the work described below including incidental materials. (vi) See the Statement of Work (SOW) in the attached addendum for a description of services required. (vii) Services shall be delivered to the USDA facility located at: Moore Air Base, Building 6420 22675 N Moorefield Rd Edinburg, TX 78541 (viii) The provision at 52.212-1 Instructions to Offerors - Commercial (Jan 2017) applies to this solicitation. (ix) The provision at 52.212-2, Evaluation -- Commercial Items, applies to this solicitation. Evaluation of Best Value will be made in accordance with FAR Part 13 where price, technical capability, and past performance are the factors. (x) The provision 52.212-3 Offeror Representations and Certifications - Commercial Items (Jan 2017) applies to this solicitation. The contractor shall be currently registered in SAM at www.sam.gov and, either complete the on-line Offeror Representations and Certifications www.SAM.gov or return a completed copy of the Offeror Representations and Certifications with their quotation. (xi) The clause at 52.212-4 Contract Terms and Conditions - Commercial Items (Jan 2017) applies to this acquisition. (xii) The clause at 52.212-5 Contract Terms and Conditions Required To Implement Statutes Or Executive Orders Commercial Items (Jan 2017), applies to this acquisition. See the attached addendum for the applicable FAR clauses identified in FAR 52.212-5 which are checked. (xiii) Additional contract requirements, provisions, and clauses: Quotation packets shall include description/brochure of dispenser to include the duration of emitting at least 50mg/day of the methyl salicylate. Quotations packets shall also include a minimum of 2 references for work similar to the work described in the addendum. References shall include a point of contact with first and last name as well as the phone number and email address (speaks to Past Performance). Additional provisions and clauses can be found in the addendum attached. (xiv) The Defense Priorities and Allocations System (DPAS) is not applicable. (xv) Quotation packets are due no later than April 12, 2018 at 1:00pm central time. Quotation packets, along with specifications and any supporting documentation shall be emailed to Jeremy Pouliot, contracting specialist, at Jeremy.pouliot@aphis.usda.gov.
