Description
The Amarillo VA Health Care System (AVAHCS) is seeking Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB) or Small Business (SB) sources capable of meeting the requirement listed below. The acquisition will be accomplished using commercial item procedures in accordance with FAR Part 13. NOTE: TO QUALIFY SDVOSB AND VOSB VENDORS MUST BE CERTIFIED IN THE VA VENDOR INFORMATION PAGES (VIP) DATABASE. The North American Industry Classification System (NAICS) is 621511. Any SDVOSB, VOSB or SB firms who wish to identify their interests and capability to provide this service must notify the Contract Specialist no later than NOON PM Central Time, April 17, 2018. Notification shall be e-mailed to Lula Hill, at lula.hill3@va.gov. Provide Cage Code and or DUNS number. In accordance with the Federal Acquisition Regulation, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Notice. SCOPE: Statement of Work Pathology Support Services for FY2019 Amarillo VAHCS The Amarillo Veterans Affairs Health Care System (AMAVAHCS) Department of Pathology & Laboratory Service requires support from an external reference laboratory. The reference laboratory shall provide, in accordance with all applicable federal, state and local regulations, laws, and ordinances, and in accordance with the specifications outlined for accreditation certification if applicable, reference laboratory testing to the e Amarillo VAHCS in accordance with the Statement of Work herein. The reference laboratory shall provide services to include the performance of analytical testing as defined by the Laboratory s test manual, the reporting of analytical test results and consultative services as required to assimilate the full scope of its laboratory operations to AMAVAHCS. All requirements and provisions defined in the specification of this solicitation will apply to any laboratory, i.e., branch, division, sub-Reference Laboratory, etc. performing reference testing on behalf of the Reference Laboratory. AMAVAHCS anticipates approximately15,000 specimens will be referred for testing in FY 2014. Reference Laboratory Responsibilities: The Reference Laboratory shall provide laboratory services to include preanalytical processing as defined in its laboratory manual, analysis, reporting of analytic results, and interpretation. The Reference Laboratory shall be responsible for providing special instructions, current lists of test with reference ranges and specimen requirements. The Reference Laboratory shall provide specimen transport containers, lids, and transport bags as required for transportation of specimens to their laboratory. The Reference Laboratory will provide requested services during normal working hours unless otherwise specified according to special services provided by the Reference Laboratory. Provide courier service with documented hazardous material shipping and handling training to transport samples in such a manner as to ensure the integrity of the specimen. The Reference Laboratory will analyze the requested samples. The Reference Laboratory will comply with reporting tests results within the specified time frame in accordance with the nature of the testing, following all regulatory requirements for reporting of patient results. All results identified as a critical or panic value will be verbally communicated immediately to AMAVAHCA Laboratory at the time of identification. All other results will be transmitted to the AMAVAHCS via the Laboratory Electronic Data Interchange (LEDI) according to the published turnaround times of the reference laboratory. The Reference Laboratory shall be in compliance with the Health Insurance Portability and Accountability Act (HIPAA) of 1996 (Public Law 104-191). The Reference Laboratory will consult laboratories on test results by telephone as needed. Reference Laboratory will provide AMAVAHCS with a means of communication to permit immediate inquiry regarding the status of pending tests. Tests shall be performed to the satisfaction of the AMAVAHCS and the Contracting Officer s Technical representative (COTR). The Reference Laboratory shall provide a monthly invoice to include: Referring dates of service, patient name, CPT code, number of CPT codes, charge of each CPT, and total charge for each patient. The Reference Laboratory shall provide a quarterly quality assurance data report to include: The specimen result turnaround time for ordered tests. This will be defined as the date a specimen was received by the Reference Laboratory until the preliminary report was available to AMAVAHCS. A summary list of all testing volume and costs for each test ordered. Reporting All reports shall include, at a minimum: Clinical Laboratory Improvement Amendment (CLIA) number Laboratory name and mailing address Case number Referring physician Patient s full name Patient s age Physicians name Date collected Date received Date reported Gross Description Pathological diagnosis Licensing and Accreditation The Reference Laboratory shall have all licenses, permits, accreditation and certificates required by law and or regulatory compliance. Personnel assigned by the Reference Laboratory to perform these services covered under this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The Reference Laboratory must be accredited by the College of American Pathologists (CAP) or have JCAHO and Clinical Laboratory Improvement Amendments (CLIA) (Public Law 100-578). Proposed Reference Laboratory must submit proof of Joint Commission on Accreditation of Healthcare Organization (JCAHO) and CLIA certifications. The Director of proposed laboratory shall be a licensed physician in the specialty or pathology. Services Required The nature, extent, and scope of reference testing does not allow for an all-inclusive list of services and associated Current Procedure Terminology (CPT) codes. The reference laboratory shall provide to the AMAVAHCS Laboratory: A list of current testing performed and any specimen requirements for each test, A current fee schedule which reflects GSA pricing, and A list of further discounted pricing for high volume tests.
