- Workers' Comp Form Experience Questionnaire Wage Determination Maps 1449 Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. 12034318Q0133 is issued as a request for quote (RFQ). This solicitation document incorporates provisions and clauses in effect through the Federal Acquisition Circular (FAC) 2005-88. This announcement constitutes the only solicitation. This solicitation is for the acquisition of a White Pine Tree Removal for the Idaho Panhandle National Forest, Coeur D'Alene Nursery. Please see the attached Request for Quote (RFQ) for additional description of the requirement and for delivery information. Submit written offers only; oral offers will not be accepted. This acquisition is a 100% small business set-aside. The Northern American Industry Classification System (NAICS) code is 562111 and the size standard of $38.5. A firm-fixed price contract will be issued. All firms or individuals responding must be registered at http://www.sam.gov. The provision 52.212-1 and 52.212-3 in clauses 52.212-4 and 52.212-5 are included as part of the acquisition. The quotes are due by 4pm PST on April 26, 2018.
