- THIS IS NOT A FORMAL REQUEST FOR PROPOSAL (RFP) AND DOES NOT COMMIT THE DEPARTMENT OF THE ARMY, US ARMY MEDICAL COMMAND, US ARMY HEALTH CONTRACTING ACTIVITY, REGIONAL HEALTH CONTRACTING OFFICE - ATLANTIC (RHCO-A) TO AWARD A CONTRACT NOW OR IN THE FUTURE This is a SOURCES SOUGHT NOTICE to determine the availability of HUBZone small businesses that have the capability, experience, and capacity to provide nonpersonal ambulance services to Fort Buchanan Puerto Rico under NAICS code 621910. Background/Scope Fort Buchanan Puerto Rico has a requirement for nonpersonal services for ambulance services for a five year period of performance. The Regional Health Contracting Office - Atlantic (RHCO-A) intends to issue a single Request For Proposal and make a single award for a firm fixed price contract. Telephone inquiries will not be accepted or acknowledged, and no feedback or evaluations will be provided to companies regarding their submissions. Capability Statement - Any potential offeror must demonstrate the following: The government will evaluate market information to ascertain potential market capacity, experience and capability in the following areas : 1) Address your ability, experience and capacity to provide personnel, vehicles, equipment, supplies, facilities, transportation, tools, materials, supervision and anything else that is necessary to perform Advanced Life Support (ALS) ambulance services at Fort Buchanan, US Army Garrison, Puerto Rico. The Ambulance Services shall be available twenty four (24) hours a day, seven (7) days a week providing service to Fort Buchanan and the surrounding area of responsibility; routine transport to include from the Rodriguez Army Health Clinic to a local hospital in/and around the metropolitan area and outside the metropolitan area. The ambulance shall be stationed at the Fire Station at Fort Buchanan. Medical personnel shall be qualified and adequately trained. Must possess Puerto Rico Department of Health Certification with control number and Puerto Rico Certificate of Authorization and License issued by the Public Service Commission. All personnel shall be bilingual English/Spanish and be able to accurately read and write in both English and Spanish. 2) If subcontracting or teaming is anticipated in order to deliver technical capability, organizations should address the administrative and management structure of such arrangements. 3) Provide the following business information: a. DUNS b. Company Name c. Company Address (including point of contact, phone number and email address) e. Type/Size of company, NAICS code as validated via the System for Award Management (SAM) The synopsis is for information and planning purposes and is not to be construed as a commitment by the Government. This is not a solicitation announcement for proposals and no contract will be awarded from this announcement. No reimbursement will be made for any costs associated with providing information in response to this announcement and any follow-up information requests. Respondents will not be notified of the results of the evaluation. All information submitted in response to this announcement must arrive on or before the closing date.
