- PROVISIONS CLAUSES IEE CERTIFICATION SPECS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, FAR Part 13 Simplified Acquisition Procedures, and 25 U.S.C 47 the Buy Indian Act as supplemented with additional information included in this notice. This request for quote constitutes the only solicitation and offers are being requested. The Indian Health Service (IHS) Intends to award a firm-fixed price contract resulting from this solicitation. The solicitation documents & incorporated provisions & clauses are those in effect through Federal Acquisition Circular 2005-97 / 01/24/2018. Set-Aside: This is set-aside 100% for Small Business Indian Economic Enterprise. Payment Terms: Net 15 Shipping Terms: FOB Destination Contract Type: Firm Fixed Price Basis of Award: Lowest price technically acceptable. Submit with Offer: - Primary point of contact to include e-mail and telephone number. - Complete pricing for the equipment list attached to include trade-in value. - Terms and conditions as applicable. - Company Dun & Bradstreet Number. - Provisions (attached) completed if not already provided in www.sam.gov. - IEE Certification (attached) completed. Prospective vendors shall be properly registered in www.sam.gov. This includes proper registration as a small business IEE vendor and subcategory claimed such as HUBZone, 8(a), Service Disabled Veteran Owned, Veteran Owned, Women-Owned, and Small Disadvantaged Business.
