- The Department of Veterans Affairs (DVA) currently intends to solicit with a single source and award a contract to provide Polymerase Chain Reaction (PCR) Equipment/System to Perform Molecular Diagnostic Patient Testing on a Cost-Per-Test (CPT) basis, as defined in the statement of work. The intended source is currently: Cepheid | 904 Caribbean Dr. | Sunnyvale, CA 94089 The statutory authority for the sole source procurement is FAR 13.106(b) Soliciting from a single source and the anticipated award date is 15 May 2018 A firm-fixed priced purchase order is anticipated. The order will have a one-year period of performance with (4) priced optional years. RESPONSES ARE HIGHLY ENCOURAGED All responses to this notice must be in writing and include the Notice # 36C24718Q9043 Intent To Sole Source in the subject line. Additionally, responses must also include: Name of the firm, point of contact, phone number, email address, DUNS number, CAGE code, identify business size (large business or a small business), and if applicable, a statement regarding small business status (including small business type(s)/certifications(s) such as SDB, 8(a), HUBZone, SDVOSB, WOSB, etc.) and the corresponding NAICS code. Identify whether your firm is interested in competing for this requirement as a prime contractor or not. Identify subcontracting, joint ventures or teaming arrangement that will be pursued, if any. Information in sufficient detail regarding previous experience (indicate whether as a prime contractor or subcontractor) on similar requirements (include size, scope, complexity, timeframe, government or commercial), pertinent certifications, etc., that will facilitate making a capability determination. All responsible sources may submit a capability statement or quotation, which shall be considered by the agency.
