This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotations (RFQ) and the solicitation number is 36C4418Q0499. The solicitation document and incorporated provisions and clauses are those attached in attachment 6 effects through Federal Acquisition Circular 2005-96 effective November 06, 2017. The associated North American Industrial Classification System (NAICS) code for this procurement is 238290, Other Services to Buildings and Dwellings with a small business size standard of $15 Million. The Department of Veterans Affairs, Erie VA Medical Center 135 East 38th Street Boulevard. Erie, PA 16504 is seeking to purchase Passenger Elevator 1 & 2 and Service Elevator 3 upgrade for Building 1. All interested companies shall provide quotations for the following: The anticipated Period of Performance will be: This upgrade is estimated to take 6 days for five hours per day and scheduled over a 3 week period beginning 15 calendar days after award for the following Contract Line Item Numbers (CLIN): CLIN 0001: Installation of Reel Closer and Interlock Assemblies (Interlock assemblies include keeper assembly, switch assembly and link assembly) of P1 Elevator. CLIN 0002: Installation of Reel Closer and Interlock Assemblies (Interlock assemblies include keeper assembly, switch assembly and link assembly) of P2 Elevator. CLIN 0003: Installation of Reel Closer and Interlock Assemblies (Interlock assemblies include keeper assembly, switch assembly and link assembly) of S3 Elevator. Place of Performance Address: Department of Veterans Affairs Erie VA Medical Center 135 East 38th Street Boulevard Erie, PA 16504 United States of America All work shall be completed in accordance with the Statement of Work (SOW). The SOW is included as Attachment 1 to this solicitation. Offerors are requested to quote Firm Fixed Price for any and all of the line items on the Schedule of Supplies and Services attached to this solicitation. The Schedule of Supplies and Services is included as Attachment 2 to this solicitation. Service Contract Act Wage Determination Number WD67-0449, Revision Number -60, dated 01-30-2018, applies to this acquisition and will be incorporated into the resulting contract. A copy of the Wage Determination is included with this solicitation as Attachment 3. Any award resulting from this solicitation will be issued on a Standard Form 1449. NOTE: To receive an award resulting from this solicitation, offerors MUST be registered in the System for Award Management (SAM) database IAW FAR 52.212-(k).
