THIS IS A SOURCES SOUGHT ANNOUNCEMENT FOR INFORMATION ONLY. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT WILL BE AWARDED FROM THIS ANNOUNCEMENT. The purpose of this notice is to conduct market research and obtain information from qualified businesses in identifying the availability of the below requirement. No solicitation is being issued at this time. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. The Manchester VA Medical Center is hosting a Vendor/Industry Day to seek responsive companies who may be interested in providing Residential Services to Homeless Veterans in the Manchester, NH area. Date: May 02, 2018 Time: 12:00pm 2:00pm (est) Location: Manchester VA Medical Center 718 Smyth Rd Manchester, NH 03104 Point of Contact: Emily Reisine 603-624-4366 (w) 603-782-6969 (m) Emily.Reisine@va.gov Contracted Emergency Residential Services (CERS) Program Program Description: The purpose of the CERS program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient therapeutic services to meet the needs of those Veterans. The Contractor will be required to provide emergency transitional housing, support and employment/training services to homeless veterans, and housing/supportive services to Veterans. It is understood that those Veterans cared for under this contract will require care and services over and above the level of room and board. This is a short-term emergency program, with length of stay limited to 90 days. There must be at least one staff member on duty on the premises or residing at the house and available for emergencies 24 hours a day, 7 days a week. Services include: Structured group activities as appropriate or by referral to community resources or VA services, where such services are accessible examples include group therapy, social skills training, Alcoholics Anonymous, Narcotics Anonymous, vocational counseling and physical activities as appropriate. Individual case management, including assisting in the development of self-care skills, adaptive coping skills and, as appropriate, vocational rehabilitation counseling, information on the scope of and referral to housing options available to the Veteran, in collaboration with VA program and community resources. Case management would ideally occur two times/week. Assistance to develop responsible living patterns, to maintain an acceptable levels of personal hygiene and grooming, and to achieve a more adaptive level of psychosocial functioning, upgraded social skills, and improved personal relationships. This may include day-to-day verbal cuing or conversation about how to best function in a social environment. Support for an alcohol/drug abuse-free lifestyle provided in an environment conducive to social interaction and the fullest development of the resident s rehabilitative potential. Delivery of the rules and regulations Veterans must abide by that are related to the alcohol/drug free environment. A program that promotes community interaction. This may mean informing Veterans of volunteer, community service, employment, and organization activities and involvement opportunities Assisting the Veterans with local transportation to scheduled meetings and appointments. The Contractor will be expected to help the Veteran access public transportation, including providing information and instructions necessary to enable Veterans to utilize public transportation. If VA staff determines that adequate public transportation is not available or appropriate for a Veteran, the Contractor shall provide transportation to and from the Manchester VAMC at least once per day for all normally operating business days (Monday through Friday), excluding Holidays. In the event that multiple transportation needs arise for Veterans in CERS program within one day, the Contractor will accommodate these multiple needs.
