Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is 36C25818Q0052 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-97 effective 01/24/18. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 1000 employees. This posting is notification of a sole source award to Stryker Corporation, sole source justification attached. FOB destination, deliver all products to: Phoenix VA Health Care System, 650 E Indian School Rd, Bldg 1, Phoenix, AZ, 85012-1892. STATEMENT OF WORK GENERAL REQUIREMENTS: The vendor shall provide Stryker Brand Name Only: 2 EA, Pi Drive Plus Motor, 2 EA Electric Handswitch - Right Handed; 2 EA 9cm Straight Attachment; 2 EA 14cm Straight Attachment; 2 EA Elite 7cm Straight Attachment; 2 EA Elite 12cm Straight Attachment; 2 EA Pi Drive Motor. TECHNICAL REQUIREMENTS: Vendor shall provide the following: 2 EA Pi Drive Plus Motor Drill that: Provides 75000 RPM Electronic Torque Feedback which increases RPM s when the drill is engaged in more demanding tasks Is tapered for better line of sight Allows for use of SD/PD and Maestro attachments Has Fixed and Steering Duraguards Allows for the use of Zyphr, Elite and MIS burs Is Fully compatible for use with our current Stryker Consolidated OR Equipment (CORE) Console system. WARRANTY: All instrumentation will be warranted for a period of one year from substantial completion or first use of the system whichever comes first. Warranty will cover instrument as well as scheduled preventive maintenance (PM). Warranty excludes instruments damaged by misuse, abuse or acts of God. SELECTION CRITERIA: The following qualifications submitted by the vendor for this project will be evaluated based upon the following Evaluation Factors: Instrument Specifications The vendor proposes instruments that meets the specifications relative to project need. Project Cost The total cost of each quote. SECURITY AND HIPPA COMPLIANCE: This item does not hold any patient information therefore a security package is not required. In accordance with Appendix A, Handbook 6500.6 Checklist blocks 6, the C&A requirements do not apply to this requirement. End, Statement of Work Interested vendors are directed to block 20 of the Standard Form 1449, the first page of solicitation 36C25818Q0052. Providing price quotes for all line items as listed in the Price/Cost Schedule is required. ***Question Submission: Interested offerors shall submit all questions concerning the RFQ at the earliest possible time to enable the Buyer to respond. Questions shall be submitted via email to: Bryan.Rogers2@va.gov. NO phone calls will be accepted. Amendments to the RFQ addressing inquiries or concerns will be posted as an amendment to the RFQ on this same website. Questions not received within a reasonable time prior to close of the RFQ may not be considered. *** New Product only; NO remanufactured or "gray market" items. All items shall be covered by manufacturer's warranty. Quote shall be valid for a minimum of 30 calendar days after due date of quotations. This solicitation requires registration by the vendor with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines.
