- drawings/measurements for parts Statement of work, contract provisions, and clauses (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This solicitation, 12639518Q0118, is issued as a request for quotation (RFQ) in accordance with FAR Part 13. (iii) The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulation (FAR) can be accessed on the Internet at http://farsite.hill.af.mil/ or at https://www.acquisition.gov/. (iv) This RFQ is set aside for small business. The NAICS code is 332313 and the small business size standard is 750 employees. (v) This requirement is the United States Department of Agriculture (USDA), Animal Plant Health Inspection Services (APHIS), Plant Protection Quarantine (PPQ) to purchase the following items in the following quantities: CLIN Description Qty Unit Price Extended Price 001 CNC Water jet cutting of caster bumper - 29qty 48" X 96" X ½" Polyethylene (See Drawing) 4,500 pcs $ $ 002 CNC Water jet cutting of caster plate - 22qty 48" X 96" X 3/16" steel plate (See Drawing) 4,500 pcs $ $ 003 CNC Water jet cutting of angle stop flat - 13qty 48" X 96" X 3/16" steel plate (See Drawing) 4,500 pcs $ $ Total Extended Price $ * Price shall include all costs needed to accomplish the work described below including shipping all parts and scrap material. (vi) See the Statement of Work (SOW) in the attached addendum for a description of services required. (vii) Services shall be rendered at contractor's facility. Finished parts shall be shipped to: 22675 N MOOREFIELD RD BLDG 6430 EDINBURG TX 78541-5033 (viii) The provision at 52.212-1 Instructions to Offerors - Commercial (Jan 2017) applies to this solicitation. (ix) The provision at 52.212-2, Evaluation -- Commercial Items, applies to this solicitation. Evaluation of Best Value will be made in accordance with FAR Part 13 where price (including costs of shipping raw materials), technical capability, and past performance are the factors. (x) The provision 52.212-3 Offeror Representations and Certifications - Commercial Items (Jan 2017) applies to this solicitation. The contractor shall be currently registered in SAM at www.sam.gov and, either complete the on-line Offeror Representations and Certifications www.SAM.gov or return a completed copy of the Offeror Representations and Certifications with their quotation. (xi) The clause at 52.212-4 Contract Terms and Conditions - Commercial Items (Jan 2017) applies to this acquisition. (xii) The clause at 52.212-5 Contract Terms and Conditions Required To Implement Statutes Or Executive Orders Commercial Items (Jan 2017), applies to this acquisition. See the attached addendum for the applicable FAR clauses identified in FAR 52.212-5 which are checked. (xiii) Additional contract requirements, provisions, and clauses: Quotations packets shall also include a minimum of 2 references for current work similar to the work described in the addendum. References shall include a point of contact with first and last name as well as the phone number and email address (speaks to Past Performance). Additional provisions and clauses can be found in the addendum attached. (xiv) The Defense Priorities and Allocations System (DPAS) is not applicable. (xv) Quotation packets are due no later than May 14, 2018 at 1:00pm central time. Quotation packets, along with specifications and any supporting documentation shall be emailed to Jeremy Pouliot, contracting specialist, at Jeremy.pouliot@aphis.usda.gov.
