Description

NORMAL SALINE SOLUTION The Department of Veterans Affairs Medical Center, VISN 4 Network Contracting Office (NCO), on behalf of the VA Pittsburgh Healthcare System (VAPHS) is conducting market research to determine interest for the purchase of Normal Saline Solution Bags. Background: The VAPHS has a requirement for one (1) case of 40 (1 liter) bags of normal saline to be delivered to Veteran s home on a monthly basis. The saline will be used to each Veteran who is a candidate for home dialysis and has been accepted into the Home Hemodialysis program Scope of Work: The Product will be ordered and direct-shipped to the VA Pittsburgh Healthcare System Veteran s home. An annual prescription with be written by a Nephrologist at VAPHS and provided to the Vendor. This prescription will enable the Vendor to provide the Veterans enrolled in the Home Hemodialysis program with the saline required for the home hemodialysis process. Packaging is 40 (1 liter) bags per case, and one case per veteran per 4-week period will be ordered. ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 New contract plus 4 option years for home delivery of cases of NSS 0.9% 1 liter saline bags (40 per case) for home dialysis patients. Cases are delivered monthly to Veterans who are on home dialysis. LOCAL STOCK NUMBER: CS 1 liter saline bags 65.00 CS ____________ ____________ GRAND TOTAL ______________ Invoicing: Invoicing will be monthly, in arrears, and reflect the actual quantities ordered during the period. The PO number shall be prominently displayed on the invoice. Privacy/Security: Vendor will deliver the saline to the Veteran s door step. Delivery: Delivery will be 30 days ARO. Delivery Location: Patient homes that are enrolled in Home Dialysis Program The North American Industry Classification System (NAICS) Code for this acquisition is 339112 Size Standard: 1,000 employees and the Classification code is 6515. THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. The Government will not pay for any information solicited.
