- This is a Market Survey for the acquisition of the STABILIZAR INDICATOR, NSN 6610- 01-326-0119. Previous source of supply data is as follows: PN: 3809445-5, CAGE: 96774; PN: 70450-01053-107, CAGE: 78286 Detailed technical data for the manufacture or acceptance of this unit is not available from the Army. Telephone responses will not be accepted. Please respond in writing or email. Suppliers capable of furnishing the STABILIZAR INDICATOR, NSN 6610- 01-326-0119, must submit the following information NLT 17 May 2018. Please complete the following questions. If a question does not apply, please enter N/A or non-applicable. (1) Refer to the market survey number in your response. (2) Please provide the following point of contact (POC) information on all responses: Company: Address: Point of Contact: Phone Number: Email Address: CAGE Code: DUNS Number: (3) Provide a brief summary of the company and its core competencies. (4) Current and/or Past Performance. Provide evidence of current/past contracts for the manufacturing of item and/or any components of the time, or similar type effort with the DoD. The vendor shall submit contract number, point of contact (POC), e-mail address, phone number, a brief description of your direct support of the effort, their part number, NSN, or nomenclature, and the date of the last manufacturing effort of the same item for which they have production experience. Again, Government contract numbers, if applicable, shall be provided. (5) Describe the qualifications of your engineering staff who have experience in the design and production of this item or components similar in complexity and technology to what is used in this item. (6) Describe your company's experience in identifying the test equipment for testing this item or components similar in complexity and technology to what is used in this item. (7) Describe your company's experience in developing or updating a Technical Data Package. (8) Does your company maintain a Program and Assessment tool to monitor and analyze the availability of components to allow clear and precise tracking of all obsolescence issues? If so, please elaborate on the Program and Assessment tool(s). (9) Describe your company's experience in identifying the alternatives for the unavailability of obsolete materials or components to mitigate obsolescence issues. (10) Describe your company's experience in identifying the alternatives for the unavailability of obsolete materials or components to mitigate obsolescence issues. (11) Describe your company's experience in performing and completing First Article Testing (FAT) requirements for this item or components similar in complexity and technology to what is used in this item within a specified timeframe. (12) Discuss your company's capability in meeting US Government contracting delivery requirements. (13) The response must include documentation such as test results, copy of purchase order if the supplier was a vendor to a listed source, or a statement from a listed source demonstrating that the supplier can, in fact, supply the Army with a unit that will be physically and functionally interchangeable with the units from a previous supplier. The successful supplier may be required to warrant that the items supplied will satisfactorily perform when used for the purpose intended by the Army. (14) State the warranty of the items that will be supplied. (15) State whether the company you represent is foreign or American owned. (16) Identification of the contractor as a Large or a Small Business. If a Small Business, please identify as an 8(a), HUB Zone, Disadvantaged, Women-Owned, or non-disadvantaged.
