- Provisions and Clauses Performance Work Statement Limited Sources Justification and Approval This is a combined synopsis/solicitation for commercial services prepared in accordance with the format prescribed in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA487718Q0223 is issued as a request for quotation (RFQ). This solicitation document incorporates provisions and clauses of those in effect through Federal Acquisition Circular 2005-97, Defense Circular Number 20171228, and Air Force Acquisition Circular 2017-1003. This procurement is being issued as a small business set aside with a Limited Source Justification and Approval (See attachment). The North American Industry Classification System Code is 481219 with a size standard of $15.0 Million or less. The right to make multiple or no award is reserved in the event it is advantageous to the Government to do so. The contractor shall provide the following Contract Line Item Number (CLIN): CLIN 0001 - (QTY: 15 hrs) Civilian Airlift and Rigger Support Period of Performance: 24 June 2018 - 29 June 2018, see projected days on attached PWS QUOTE MUST INCLUDE: The name of the company of the Air Carrier being used for the Airlift and Rigger Services IAW with the attached PWS. Quoted as one line item in the format of price per hour. SEE ATTACHED PWS FOR ADDITIONAL INFORMATION THE AIRLIFT PORTION OF THIS REQUIREMENT IS A LIMITED SOURCE REQUIREMENT IN ACCORDANCE WITH THE ATTACHED J&A SEE ATTACHED FOR ALL APPLICABLE CONTRACT CLAUSES AND PROVISIONS QUOTES WILL BE EVALUATED ON TECHNICAL ACCEPTABILITY AND LOWEST PRICE. Only quotes reflecting requirement fulfillment in its entirety, as expressed in the attached PWS document, will be considered for award. The requirement will be fulfilled using a Firm Fixed Price contract. Any questions pertaining to this acquisition shall be submitted in writing no later than Tuesday, 15 May 2018 by 4:00 PM Arizona Local Time. Written questions shall be sent to SrA Matthew Killian at matthew.killian.3@us.af.mil and 2d Lt Nathan Mueller at nathan.mueller.6@us.af.mil. NO questions will be considered after close of questions due date. Questions will be addressed no later than Wednesday, 16 May 2018 by 4:00 P.M., Arizona Local Time. It is the responsibility of the offeror to view the website for any changes or amendments to this RFQ. Quotes will be due to the 355th Contracting Squadron, 3180 S. Craycroft Rd. Davis-Monthan AFB, AZ 85707, via email Tuesday, 22 May 2018, 2:00 P.M, Arizona Local Time.
